针灸配合滋阴补肾兼化痰祛瘀法在海南省高碘饮食致结节性甲状腺肿中的防治作用

Prevention and cure effect of acupuncture combined with nourishing yin and tonifying kidney and removing phlegm and blood stasis in nodular goiter caused by high iodine diet in Hainan Province

目的探讨针灸配合滋阴补肾兼化痰祛瘀法在海南省高碘饮食致结节性甲状腺肿中的防治作用。方法于海南省采用多阶段整群随机法抽取有结节性甲状腺肿患病倾向的研究对象270名,并按照随机数字表法分为观察组和对照组,各135名。两组均进行饮食调整,观察组在此基础上给予滋阴补肾兼化痰祛瘀法预防结节性甲状腺肿。1个月后对两组结节性甲状腺肿发生率进行统计,并将观察组和对照组发生结节性甲状腺肿的患者分别定名为联合组(针灸+滋阴补肾兼化痰祛瘀法+优甲乐片)和西药组(优甲乐片),观察两组患者的总体疗效、治疗前后相关指标水平变化及不良反应情况。结果观察组结节性甲状腺肿发生率24.44%显著低于对照组72.59%(P<0.05)。联合组总有效率96.97%显著高于西药组74.49%(P<0.05)。联合组治疗前后组内比较差异有统计学意义(P<0.05),西药组除IL-6外,其他指标组内比较差异无统计学意义(P>0.05)。联合组改善情况优于西药组,差异有统计学意义(P<0.05)。结论针灸配合滋阴补肾兼化痰祛瘀法对海南省高碘饮食致结节性甲状腺肿预防和治疗效果显著,安全性高,可广泛推广使用。

Objective To discuss the prevention and cure effect of acupuncture combined with nourishing yin and tonifying kidney and removing phlegm and blood stasis in nodular goiter caused by high iodine diet in Hainan Province.Methods270 subjects with prevalence of nodular goiter were selected by multi stage cluster random method in Hainan Province,who were divided into observation group and control group according to the random digital table method,with 135 cases in each group.The two groups were given diet adjustment,and the observation group were given nourishing yin and tonifying kidney and removing phlegm and blood stasis to prevent the nodular goiter based on it.The incidence rates of nodular goiter in the two groups were statistically after 1month,and the patients occurred nodular goiter in the observation group and the control group were respectively named for the combination group(acupuncture+nourishing yin and tonifying kidney and removing phlegm and blood stasis+euthyrox tables)and the western medicine group(euthyrox tables).The overall effect of the two groups of patients,the level of the relevant indicators before and after treatment and adverse reactions were observed.Results The incidence rate of nodular goiter in the observation group was 24.44%,significantly lower than that of 72.59%in the control group(P<0.05),and the total effective rate in the combination group was 96.97%,significantly higher than that of 74.49%in the western medicine group difference was statistically significant(P<0.05).There were no significant differences in the relevant indicators before and after treatment in the combination group(P>0.05),while there were no significant differences in the other indicators except for the IL-6levels before and after treatment in the western medicine group difference was not statistically signficant(P>0.05).The improvement of the combination group was better than that of the western medicine group,and the difference was statistically significant(P<0.05).Conclusion Trevention and cure effect of acupuncture combined with nourishing yin and tonifying kidney and removing phlegm and blood stasis in nodular goiter caused by high iodine diet in Hainan Province are remarkable,with high security,which can be widely used.
