改良布朗氏架在股骨粗隆间骨折保守治疗中的应用

Improved Brown's Frame in the Application of the Conservative Treatment of Intertrochanteric Fracture

目的：设计一种针对股骨粗隆间骨折保守治疗的固定装置。方法：选取2013年5月-2015年5月住院股骨粗隆间骨折患者20例，其中男11例，女9例；年龄63~87岁，平均73岁。所有患者行骨牵引后，患肢放置于改良布朗氏架上牵引固定。评价装置在股骨粗隆间骨折保守治疗中的应用效果。结果：20例均获得随访。根据改良Harris髋关节评分为61~90分，平均80.9分。其中优2例，良9例，可6例，差3例。无髋内翻发生。下肢短缩（0.69±0.81）cm。结论：改良布朗氏架的设计符合生物力学，对股骨粗隆间骨折造成的髋内翻和下肢短缩畸形有效，适合推广应用。

Objective：To design a fixing device for the conservative treatment of intertrochanteric fracture.Method：From May 2013 to May 2015,20 patients with intertrochanteric fractures in the hospital were selected,there were 11 males and 9 females with the average age of 73 years old（63-87 years old）. Bone traction for all of the patients,limbs were put on the improved Brown’s frame with traction and fixation.The application effect of the devices in the conservative treatment of intertrochanteric fracture was evaluated.Result：All the patients were followed up.Harris Hip score from 61 points to 90 points with an average of 80.9 points.2 patients were excellent.9 patients were good.6 patients were fair.3 patients were poor.There was no coxa vara was found.Shortening of limbs was （0.69±0.81）cm. Conclusion：The design of improved Brown’s frame is accordant with biomechanics.It is more efficient for coxa vara and shortening of limbs caused by fracture of the femoral intertrochanteric.It is suitable for popularization and application.
