串联质谱法对早产儿TPN营养支持代谢的研究

摘要 目的:探讨串联质谱法在早产儿营养支持后代谢研究中的价值。方法:选择本院2014年6月-2015年9月收治的、胎龄为28~33周的62例早产患儿为研究对象,实施全胃肠外营养(TPN)后48-72 h手指采血,采用串联质谱法(tandem mass spectrometry,MS-MS)选择性分析血液样本中氨基酸谱和酰基肉碱谱及其浓度。结果:11例标本检出阳性结果,其中高鸟氨酸血症2例,高酪氨酸血症3例,高丙酸血症2例,高甲基丙二酸血症1例,肉碱转运障碍2例,高异戊酸血症1例。结论:串联质谱技术可检测出早产儿TPN营养支持后所代谢物质的浓度水平,并指导临床营养支持方案。 Objective:To explore the value of tandem mass spectrometry in the study on nutrient metabolism in preterm infants.Method:Sixty-two preterm infants with gestational age of 28-33 weeks admitted to our hospital from June 2014 to September 2015 were selected as the objects,after the implementation of TPN for 48-72 hours,their finger blood were sampled,then the concentration of amino acids and acylcarnitines spectrum in blood smear were analyzed by tandem mass spectrometry(MS-MS).Result:Eleven cases of all the preterm infants revealed a positive results,2 cases of hyperornithinemia,3 cases of tyrosinemia,2 cases of propionic acidemia,1 case of methylmalonic acidemia,2 cases of carnitine transport barriers,and 1 case of isovaleric acidemia.Conclusion:The tandem mass spectrometry technique can detect the nutrition metabolic substance levels in preterm infants and guide the clinical nutritional support program.
