摘要
目的了解90岁以上下肢动脉闭塞症患者的临床特征以及他们在血压和血糖方面的特点。方法选择2014年1-12月在我院老年病房住院的90岁以上患者145例,根据彩色多普勒超声分为动脉闭塞组75例,非动脉闭塞组70例。2组行24h动态血压监测,比较24h平均收缩压、24h平均舒张压、24h平均脉压、昼间平均收缩压、昼间平均舒张压、夜间平均收缩压、夜间平均舒张压以及一般临床资料及血生化。结果动脉闭塞组高血压、超重或肥胖、体质量指数、服用降糖药物、降压药物比例明显高于非动脉闭塞组[88.0%vs 74.3%,48.0%vs30.0%,(24.0±3.6)kg/m2 vs(22.3±3.9)kg/m2,54.1%vs 28.0%,86.4%vs 69.2%,P〈0.05]。动脉闭塞组24h平均收缩压、昼间平均收缩压、24h平均脉压明显高于非动脉闭塞组[(133.5±15.7)mmHg vs(127.5±14.1)mmHg,(133.4±15.9)mmHg vs(127.0±14.2)mmHg,(69.5±14.4)mmHg vs(63.7±10.6)mmHg,1mmHg=0.133kPa,P〈0.05,P〈0.01]。结论 90岁以上人群下肢动脉闭塞症发生率明显升高。建议90岁以上人群仍需要积极控制血压以及体质量指数。
Objective To study the characteristics of blood pressure and blood glucose in peripheral arterial occlusive disease(PAOD)patients aged over 90 years.Methods One hundred and fortyfive patients aged over 90 years admitted to our hospital from January 2014 to December 2014 were divided into PAOD group(n=75)and PAOD-free group(n=70)according to their ultrasonography.Their 24 hambulatory blood pressure,24 haverage systolic blood pressure(SBP),24 h average diastolic blood pressure(DBP),24 haverage pulse pressure(PP),day average SBP,day average DBP,clinical data and blood biochemical parameters were recorded and compared.Results The incidence of hypertension and overweight or obesity,the rate of administered antidiabetic and antihypertensive drugs,and BMI were significantly higher in PAOD group than in PAOD-free group(88.0%vs 74.3%,48.0%vs 30.0%,54.1%vs 28.0%,86.4%vs 69.2%,24.0±3.6kg/m2 vs 22.3±3.9kg/m2,P〈0.05).The 24 haverage SBP,day average SBP,and 24 haverage PP were significantly higher in PAOD group than in PAOD-free group(133.5±15.7 mmHg vs127.5±14.1 mmHg,133.4±15.9 mmHg vs 127.0±14.2 mmHg,69.5±14.4 mmHg vs63.7±10.6mmHg,P〈0.05,P〈0.01).Conclusion The incidence of PAOD is high in patients aged over 90 years.It is thus necessary to control their BMI and blood pressure.
