摘要 目的建立带骨骺缝匠肌髂骨瓣修复髋关节发育不良中髋臼缺损的动物模型,并探讨骨形态发生蛋白II型(BMP-2)和血管内皮细胞生长因子(VEGF)在修复过程中的表达及意义。方法在制备16只日本大耳白兔缺损左侧髋臼上缘骨质制备髋关节发育不良模型的基础上,采用带骨骺缝匠肌髂骨瓣对缺损进行修复,术中观察骨瓣血运情况和拉伸程度,术后1周、2周、4周和8周观察大体步态和髋关节活动度,并处死动物取髋臼缺损修复部位的组织进行HE染色以及BMP-2和VEGF的免疫组化染色。结果术中带骨骺缝匠肌髂骨瓣血运良好,缝匠肌拉伸程度适中;术后8周时行动步态和髋关节活动度接近正常;术后1周可见带骨骺缝匠肌骨瓣与髋臼骨组织间存在明显间隙,两者软骨层间距较大,2周和4周骨瓣软骨层与髋臼软骨层间距缩短,至8周骨瓣软骨层与髋臼软骨层几乎融合;BMP-2和VEGF在术后4周时处于表达高峰期,并明显高于其他时间点(P<0.05),1周与2周、8周之间差异具有统计学意义(P<0.05),而其他时间点间未见明显差异(P>0.05)。结论 BMP-2和VEGF的表达增加参与了兔髋臼缺损修复的病理生理过程。 Objective Establish an animal model of the sartorius iliac flap with epiphysis for the repair of an acetabular defect in developmental dysplasia of the hip( DDH). Then,discuss the expression of BMP- 2 and VEGF during the repair of an acetabular defect and its significance. Methods A unilateral acetabular bone defect were applied to 16 Japanese white rabbits. Then,an acetabular defect was repaired with sartoriusepiphysial iliac flap. During operation,the blood supply and activity length of the sartoriusmuscular iliac flap were monitored. The general gait and hip activity rang were also examined after operation. Animals were killed to harvest the acetabular bone tissues in postoperative weeks 1,2,4 and 8. HE staining and the immunohistochemical staining of BMP- 2 and VEGF were performed with the samples. Results Blood supply of the sartorius iliac flap was good,and activity length was enough. The general gait and hip activity rang were nearly normal at 8 weeks after operation. It showed an obvious gap between the sartorius iliac flap with epiphysis and acetabular bone tissue at 1 week group. The distance between the sartorius iliac flap ' s cartilage and acetabular cartilage became shorter at 2 weeks and4 weeks. It is until 8 weeks that the flap cartilage and acetabular cartilage were almost fused. The expression of BMP- 2 and VEGF reached the peak at 4 weeks after operation,which is higher than other time points( P 0. 05). Also,1 week group and 2,8 weeks group respectively had statistically significant differences( P 0. 05). Between other time points,it demonstrated no significant difference( P 0. 05). Conclusions The increasing expression of BMP- 2 and VEGF participates in the pathophysiological process of repairing acetabular defects in rabbits using sartoriusepiphysial iliac flap.
