84例钝针扣眼穿刺法的观察及护理体会

Observation and nursing experience of 84 cases undergoing button hole cannulation with blunt needle

目的分析动静脉内瘘扣眼穿刺法的效果，总结穿刺经验及对并发症的护理体会。方法用视觉模拟评分法评估患者疼痛评分，记录并分析止血时间、穿刺成功率以及预防动脉瘤、内瘘感染、皮下血肿的效果。结果84例实施扣眼穿刺法的患者有76例（90．5％）疼痛减轻，76例（90．5％）止血时间缩短，0例形成动脉瘤，1（1．2％）例内瘘栓塞，2例（2．4％）内瘘感染；穿刺3700例次，成功3678（99．4％）例次；皮下血肿7（0．2％）例次。结论扣眼穿刺法可降低穿刺并发症的发生，是一种安全、可靠的穿刺方法。

Objective To analyze the effect of button hole cannulation with blunt needle on autogenous arteriovenous fistulaes （AVFs） and to summarize the cannulation experience and nursing care for related complications. Methods visual analogue scale was used to assess the patients＇ pain. The bleeding time, the successful rate of cannulation, and complications of aneurysm, fistula infection, subcutaneous hematoma were recorded. Results 84 patients underwent button hole cannulation with blunt needle. 76 （90.5%） of them reported less pain and shorter bleeding time. No aneurysm was reported. But 2 cases （2.4%） got fistula infection. Cannulated 3 700 times, and 3 678 times （99.4%） were successful; and 7 times （0.2%） led to subcutaneous hematomas. Conclusions Generally, button hole cannulation with blunt needle is safe and reliable.
