摘要
目的探讨卒中后抑郁患者的认知功能与血浆同型半胱氨酸水平的关系。方法选取2014年1月至2015年8月我院收治的80例脑卒中后抑郁患者(A组)、80例脑卒中后未抑郁患者(B组)和在院体检的正常人80例(C组)作为临床研究对象。观察比较三组的认知功能、痴呆量表、血清Hcy评测结果。结果 A组的认知功能评分明显低于B组与C组(P<0.05);B组与C组在定向力和语言能力方面差异无意义(P>0.05),在注意力和记忆力方面C组显著优于B组(P<0.05)。A组、B组、C组的CDR评分分别为(2.74±0.65)分、(2.24±0.52)分、(1.84±0.48)分,A组评分显著高于B组和C组,B组显著高于C组(均P<0.05)。A组、B组、C组的血清Hcy水平分别为(24.54±5.75)μmol/L、(18.25±5.56)μmol/L、(13.47±5.48)μmol/L,A组明显高于B组和C组,B组显著高于C组(均P<0.05)。结论脑卒中后抑郁患者的认知功能水平与Hcy水平有关,并且随着Hcy水平的升高而下降,临床上应及时对血清Hcy水平进行检查和控制。
Objective To explore the relationship between the cognitive function and plasma homocysteine (Hcy) level of patients with post-stroke depression. Methods From January 2014 to August 2015, 80 cases &post-stroke depression (group A), 80 cases of stroke patients without depression (group B), and 80 cases of normal subjects with physical examination (group C) were selected as the research objects. The cognitive function, dementia scale score and serum Hcy level were compared among 3 groups. Results The cognitive function of group A was significantly lower than that of the other two groups (P 〈0.05); No statistical difference was found between group B and group C in the orientation and language ability (P〉0.05), while group C was superior to group B in the attention and memory (P〈0.05). The CDR scores &group A, group B and group C were (2.74 ±0.65) points, (2.24 ±0.52) points and (1.84± 0.48) points respectively; Group A was significantly higher than the other two groups, and group B was significantly higher than group C (P 〈0.05). The serum Hcy level of group A, group B and group C were (24.54 ±5.75)μmol/L, (18.25 ±5.56)μmol/L and (13.47 ±5.48)μmol/L respectively; Group A was significantly higher than the other two groups, and group B was significantly higher than group C (P 〈0.05). Conclusions The cognitive function of patient with post-stroke depression is related to Hcy level, and decreases with the increase of Hcy level. Clinicians should timely check and control the serum Hcy level.
