摘要
目的研究骨科损伤控制在不稳定盆骨骨折治疗中的应用价值。方法选取我院2013年4月~2015年4月50例不稳定性盆骨骨折患者作为研究对象,随机分为两组,每组25例。对照组行常规治疗,观察组行骨科损伤控制治疗。治疗结束后随访半年,观察治疗后骨折愈合、功能恢复及并发症发生情况。结果观察组治疗总有效率、生活自理率及Majeed评分分别为92%、84%及(86.43±5.25)分,均显著高于对照组,差异具有统计学意义(P〈0.05)。观察组术后急性呼吸窘迫综合征(ARDS)、感染及肠梗阻等并发症发生率,显著低于对照组,差异具有统计学意义(P〈0.05)。结论骨科损伤控制可显著改善骨盆骨折愈合质量,促进局部功能恢复,提高患者自理能力,且可降低术后并发症发生率。
Objective: To investigate the feasibility of damage control orthopedics(DCO) in treatment of unstable pelvis fracture. Method: Totally, 50 patients with unstable pelvis fractures as the research, who were treated in the first people's hospital of Chengdu between April 2013 and April 2015. They were randomly divided into test group and control group with 25 patients in each group. Patients in control group were only treated routinely, and patients in test group were performed with damage control orthopedics. Follow-up for half a year, we observed their recovery, such as fracture healing, functional recovery, and the incidence of complications. Results: The total effective rate(92%), the rate of life-independence(84%), and Majeed score[(86.43±5.25)] were significantly higher in test group than in control group, and statistically significant differences(P〈0.05).The incidence of complications including that acute respiratory distress syndrome(ARDS), infection, and intestinal obstruction were significantly lower in test group than in control group, and statistically significant differences(P〈0.05). Conclusions: Damage control orthopedics can significantly improve the quality of pelvis fracture healing, promote the recovery of local function, increase patient's life self-care ability as well as reduce the incidence of postoperative complications.
