重症急性胰腺炎致急性呼吸窘迫综合征的监护观察

Observation of severe acute pancreatitis induced acute care respiratory distress syndrome

目的：探讨重症急性胰腺炎致急性呼吸窘迫综合征的临床特征，并对早期呼吸支持对该类患者的治疗价值进行分析。方法随机选取濮阳市人民医院2012年9月至2015年1月收治的重症急性胰腺炎合并急性呼吸窘迫综合征患者54例作为观察组，另选取同期我院收治的单纯重症急性胰腺炎患者54例作为对照组，对两组患者入院时的临床情况进行观察对比，并采用早期呼吸支持对观察组患者进行治疗，对其治疗效果进行回顾性分析。结果观察组入院时的胰腺 CT 评分、APACHE-Ⅱ评分、呼吸频率、胰腺感染率均明显高于对照组，差异有统计学意义（P〈0.05）；观察组经常规治疗联合早期呼吸支持后，其各项动脉血气观察指标较治疗前均有显著改善，差异有统计学意义（P〈0.05）。结论全面的入院检查和评估以及早期呼吸支持对于保障重症急性胰腺炎致急性呼吸窘迫综合征患者的生命安全，促进其及早康复具有积极的临床意义。

Objective To investigate the clinical characteristics of severe acute pancreatitis caused by acute re-spiratory distress syndrome,and therapeutic value in patients with early respiratory support that type of analysis. Methods Randomly selected 54 cases with severe acute pancreatitis combined with acute respiratory distress syndrome in people’s hospital of Puyang from September 2012 to January 2015 as the observation group,and chose another 54 patients with severe acute pancreatitis as control group. The clinical data of the patients in the two groups when they were admitted to hospital were compared. The patients in observation group were treated with early respiratory support,and retrospectively analyzed the treatment effects. Results Pancreas CT score,APACHE-Ⅱ score,respiratory rate,pancreatic infection rates of the patients in observation group when admitted in hospital were significantly higher than that of control group（P〈0. 05）;after early respiratory support in the observation group,the arterial blood gas before treatment outcome measures were significantly improved（P〈 0. 05）. Conclusions Comprehensive admission examination and assessment and early respiratory support for the protection of severe acute pancreatitis caused by acute respiratory distress syndrome patient safe-ty,so promoting their early recovery has a very positive clinical significance.
