原发性高血压与心律失常的相关性

Relation between essential hypertension and arrhythmia

目的：探讨原发性高血压与心律失常之间的相关性及其机制。方法选择襄汾县人民医院确诊为原发性高血压的96例患者为研究对象，根据有无心律失常及并发症对其进行分组，分析原发性高血压患者并发心律失常与左室舒张末期内径（EDD）、左房内径（LA）、舒张早期二尖瓣血流速度与舒张晚期二尖瓣血流速度（E/A）比值等指数的关系及原发性高血压是否合并并发症与心律失常的关系。结果原发性高血压并发心律失常组患者EDD、LA水平明显高于单纯原发性高血压组，E/A水平低于单纯原发性高血压组，差异有统计学意义（ P﹤0.05）；原发性高血压合并左心室肥大者心律失常发生率显著高于未合并左心室肥大者，差异有统计学意义（ P﹤0.01）；原发性高血压合并心肌缺血者心律失常发生率显著高于未合并心肌缺血者，差异有统计学意义（ P﹤0.01）。结论原发性高血压合并心律失常与EDD、LA、E/A比值等指标有关，且出现心律失常的患者常伴发心脏靶器官损害。

objective To investigate the correlation and mechanism betWeen essential hyperten-sion and arrhythmia. Methods The clinical data of 96 patients Who Were diagnosed as essential hyper-tension Were retrospectively analyzed,patients Were divided into subgroups according to Whether com-bined With arrhythmia or complications. The relationship betWeen patients With essential hypertension combined With arrhythmia and heart rate variability index such as EDD,lA,E/A ratio Was analyzed, and the relationship betWeen essential hypertension patients Whether combined With complications and ar-rhythmia Was analyzed. Results The levels of EDD,lA in patients With essential hypertension com-bined With arrhythmia Were significantly higher than those in patients With essential hypertension,E/A level Was loWer than that in patients With essential hypertension,the difference Was significant（ P ﹤0. 05 ）;the incidence of arrhythmia in patients With essential hypertension combined With left ventricular hypertrophy Was significantly higher than that in patients Without left ventricular hypertrophy,the differ-ence Was significant（ P﹤0. 01 ）;the incidence of arrhythmia in patients With essential hypertension com-bined With myocardial ischemia Was significantly higher than that in patients Without myocardial ischemia, the difference Was significant（ P ﹤0. 01 ）. Conclusions There is correlation betWeen essential hyper-tension combined With arrhythmia and heart rate variability index such as EDD,lA,E/A ratio,and pa-tients combined With arrhythmias is often accompanied With target organ damage.
