无创呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭的临床观察 被引量:7

Clinical observation of noninvasive ventilation in treatment of chronic obstructive pulmonary disease complicated with respiratory failure

摘要 目的探讨无创呼吸机在慢性阻塞性肺疾病(COPD)并发呼吸衰竭中的临床治疗效果。方法选取2012年6月至2014年6月收治的82例COPD合并呼吸衰竭患者作为探讨对象,按照随机数字表法将其分为对照组和观察组,对照组患者给予传统常规基础治疗,观察组患者在此基础上联合使用呼吸机治疗,并比较两种治疗方法的临床治疗效果。结果经过有效治疗后,观察组治疗总有效率为92,7%,对照组总有效率为73.2%,两组比较差异有统计学意义(P〈0.05);观察组ICU时间、总住院时间及血气指标改善情况均明显优于对照组,两组比较差异有统计学意义(P〈0.05)。结论无创呼吸在治疗慢性阻塞性肺疾病合并呼吸衰竭等方面临床效果较好,且能有效的纠正CO2潴留以及显著降低患者治疗时间,该方法更易被患者所接受。 Objective To investigate the clinical effects of non-invasive ventilation on COPD complicated with re- spiratory failure. Methods Eighty-two patients with COPD complicated with respiratory failure were selected to be study objects, who were admitted in the People' s Hospital of Luyi,from June 2012 to June 2014. They were divided into control group and observation group according to random number table. Patients in control group received traditional, conventional and basic treatment, while patients were treated with non-invasive ventilator on the b/tsis of above therapy in observation group, the clinical treatment effects of two methods were compared. Results After effective treatment, the total effective rate of patients in observation group was 92. 7%, which of patients in control group was 73.2%. The difference was signifi- cant(P 〈0.05). Time of ICU, total hospital stay and improved conditions of indexes of blood gas were obviously better than those in the control group. The differences were significant (P 〈 0. 05). Conclusions The clinical effects of noninva- sive breathing on COPD complicated with respiratory failure are better, which can effectively correct CO2 retention and sig- nificantly reduce treatment time. This method is more easily accepted by patients.
