ω-3不饱和脂肪酸乳剂对胃十二指肠溃疡穿孔患者术后炎性反应的影响 被引量:4

摘要 目的:探讨ω-3不饱和脂肪酸乳剂在胃十二指肠溃疡穿孔修补术后炎性反应的影响。方法:将90例胃十二指肠溃疡穿孔修补术后患者按照随机数字表法随机分为观察组和对照组,每组45例。术后两组患者使用等氮、等热量的TPN支持,观察组添加ω-3不饱和脂肪酸乳剂。分别检测术前1天,术后第3天和第5天外周血白细胞计数(WBC)、中性粒细胞计数,降钙素原(PTC)、以及手术后感染性并发症。结果:不同检测时间的白细胞计数之间差异有统计学意义(F=21.314,P=0.000);不同检测时间、不同分组的中性粒细胞计数之间均有统计学意义(P<0.05),检测时间和不同处理方式之间存在交互作用(F=6.412,P=0.021);不同检测时间、不同分组的降钙素原之间均有统计学意义(P<0.05),检测时间和不同处理方式之间存在交互作用(F=6.731,P=0.027);两组患者感染性并发症的感染率之间无统计学差异(χ2=1.047,P=0.306)。结论:ω-3不饱和脂肪酸乳剂对胃十二指肠溃疡穿孔修补术后的患者明显抑制炎症,降低术后炎症反应,减轻患者痛苦。 Objective: To study the effect of ω-3 polyunsaturated fatty on inflammatory reaction of patients with gastroduodenal ulcerperforation after operation. Methods:Ninety patients were randomly divided into study group(n=45) and control group(n=45).All the pa-tients were assigned to receive total parenteral nutrition with the equal nitrogen and calory,those in study group received ω-3 polyunsaturatedfatty additionally. white blood cell count(WBC),total neutrophils count, serum procalcitonin(PCT) were determined preoperatively one dayand the third day, fifth day postoperatively, the rate of postoperative infection complications were determined. Results: There was statisticallysignificant difference between the 2 groups in different testing time of White blood cell count(WBC)(F=21.314,P=0.000); There was statis-tically significant difference between the 2 groups in different testing time and different group of total neutrophils count(P〈0.05),There wasinteraction effects of different testing time and processing method(F=6.412,P=0.021).There was statistically significant difference betweenthe 2 groups in the levels of serum procalcitonin(PCT)(P〈0.05),There was interaction effects of different testing time and processing method(F=6.731,P=0.027);there was no significant difference between the 2 groups in the rate of postoperative infection complications(χ^2=1.047,P=0.306).Conclusion: In postoperative patients with gastroduodenal ulcer perforation,omega-3 fatty acid-supplemented parenteral nutritioncan inhibit the inflammatory factors, decrease postoperative inflammation and relieve patients' pain.
