盐酸氨溴索在肺癌合并阻塞性肺炎中的临床应用

The clinical application of ambroxol hydrochloride in patients with lung cancer merge obstructive pneumonia

目的观察盐酸氨溴索治疗肺癌合并阻塞性肺炎的临床疗效。方法收集肺癌合并阻塞性肺炎患者,随机分为对照组和研究组,对照组给予常规治疗,研究组给予联合盐酸氨溴索治疗,对比分析两组在临床症状好转时间、炎症指标、血气分析指标及影像学上病灶的改变情况。结果研究组患者的咳嗽、咳痰,肺部啰音好转时间短于对照组;所有患者经治疗1周后白细胞总数(WBC)、中性粒细胞数(N)、C反应蛋白(CRP)、降钙素原(PCT)、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)、氧合指数(PaO2/FiO2)指标均较治疗前明显下降,研究组治疗后指标改善大于对照组;经治疗1周后研究组病灶明显吸收例数较对照组多;研究组抗菌药物使用时间和住院时间均较对照组少。结论对于合并阻塞性肺炎的肺癌患者联合盐酸氨溴索治疗,能更好的缓解患者症状、控制炎症、提高氧合、促进病灶吸收,减少抗菌药物的使用时间和住院时间。

Objective To observe the clinical effect of patients with lung cancer merge obstructive pneumonia treated by ambroxol hydrochloride.Methods Patients with obstructive pneumonia combine lung cancer were collected,and randomly divided into control and study groups.The control group was given conventional treatment,and the study group was given ambroxol hydrochloride treatment.the two groups change in clinical improvement time,inflammatory markers,blood gas analysis and imaging were compered.Results improved time of patients of the study group were shorter than the control group in the aspect of cough,expectoration,pulmonary rales.The parameters of total white blood cell（WBC）,neutrophil（N）,C-reactive protein（CRP）,procalcitonin（PCT）,arterial oxygen tension（PaO2）,arterial oxygen saturation（SaO2）,oxygen composite index（PaO2/FiO2）,were significantly lower.after patients receiving treatment a week.The indicators of the study group improced more than the control group after treatment.After treatment week,cases of absorbing lesions of the study group were more than the control group.Using of antibiotics and hospital stay of the study group spent less time than the control group.Conclusion The treatment of Ambroxol on obstructive pneumonia in patients with lung cancer could alleviate the symptoms,control inflammation,improve oxygenation,promote focus absorption and reduce the use of antibiotics and hospital stay.
