经皮子宫动脉栓塞术治疗胎盘植入产后出血的护理

Nursing of Postpartum Hemorrhage on Placenta Accreta by Uterine Artery Embolization Treatment

目的探析子宫动脉栓塞术用于胎盘植入产后出血的护理。方法选取我院收治的胎盘植入产后出血的患者62例,按照入院时间顺序分为对照组和观察组各31例。对照组给予传统清宫手术清除残存的胎盘组织,观察组子宫动脉栓塞术治疗后清宫治疗,并术后予以护理干预,对两组临床治疗与护理效果进行评价。结果观察组的手术时间(104±18)min和出血量(1 410±982)m L低于对照组的(141±12)min和(1 638±1 024)m L(均P<0.05),术后观察组并发症9.68%(3/31)明显低于对照组25.81%(8/31)(P<0.05)。结论采用子宫动脉栓塞术并予以相应护理干预治疗胎盘植入产后出血,能缩短手术时间和降低失血量,减少术后并发症的发生,护理干预能缓解患者的不良情绪,提高患者生活质量,值得临床推广和应用。

Objective To investigate the treatment and nursing of postpartum hemorrhage on placenta implantation after uterine artery embolization with complete curettage of uterine cavity. Methods 62 patients with placenta accreta postpartum hemorrhage were selected in our hospital and divided into control group and observation group according to the order of admission with 31 cases in each group.The patients in control group were treated by the conventional surgical method with complete curettage of uterine cavity,and the observation group had the operation after uterine artery embolization treatment.Nursing intervention was used to the observation group.Then the effect of clinical treatment and nursing care was evaluated in the two groups. Results The operation time （104 ±18）min and the amount of bleeding（1 410 ±982）mL in observation group were lower than control group （141 ±12）min and（1 638 ±1 024）mL （all P 〈0.05 ）, and postoperative complications in observation 9.68% （3 /31 ） was lower than that of the control group 25.81 % （8 /31 ）（P 〈0.05）. Conclusion It could shorten the operation time and reduce blood loss and incidence of postoperative complications by complete curettage of uterine cavity after uterine artery embolization in the treatment postpartum hemorrhage of placenta accrete.The operation and the corresponding nursing intervention could relieve patients negative emotions and improve patients＇quality of life,so it was worthy of promotion and application in clinical practice.
