术前体位试摆放对全麻侧卧位患者就医体验的影响

Effect of trying a good posture before the operation on the experience of patients undergoing general anesthesia in a lateral decubitus position

目的探讨全麻侧卧位手术患者主动参与术前体位试摆放的应用效果,通过施行体位试摆放在手术室开展个性化护理,缓解患者对手术的应激反应,增强自信心,保证手术体位的舒适度,提高患者满意度,满足患者自尊及安全的需要,改善患者的就医体验。方法选取太和县人民医院2013年3月—2015年1月手术室80例全麻侧卧位手术患者,随机分为实验组与对照组。实验组40例实行术前个性化体位试摆放护理,对照组40例实行传统式护理。采用向患者发放自制满意度问卷调查表对2组患者作满意度调查,调查者逐项解释调查内容,对比评价效果。结果通过个性化术前体位试摆放,增进了护患之间的沟通、信任,提高了患者的满意度。实验组患者在受压皮肤及器官、肢体感觉2项满意度(97.5%,97.5%)明显高于对照组(70%,50%),2组满意度差异有统计学意义(P<0.01);实验组满意度调查中不满意4项(0.67%),对照组满意度调查中不满意49项(8.16%),2组差异具有统计学意义(P<0.01)。结论通过实施个性化体位试摆放,结合患者满意度调查,可以发现工作中的不足,持续改进;鼓励患者主动参与,及时知晓患者的就医体验,使患者术中更安全舒适,有利于手术室优质护理工作的开展。

Objective To investigate the effect of trying a good posture before the operation on the experience of patients undergoing general anesthesia in a lateral decubitus position, carry our individualized nursing in the surgery room, and improve the patients satisfaction. Methods Total 80 patients undergoing general anesthesia in a lateral decubitus position in our hospital between March, 2013 and January,2015 were randomly divided into experimental group and control group with 40 cases in each group. The personalized trying a good posture before the operation was performed in the experimental group, while the control group received the traditional nursing. The self-made questionnaire was used to survey patient sat- isfaction. Results The satisfaction of patients in the experimental group for ＂pressurized skin and organ＂ （97.5%） and ＂the physical sensations＂ （97.5%） was significantly higher than 70% and 50% in the control group,the difference was statistical significant （P 〈 0. 01 ） ;The patients in the experimental group were not t satisfied with 4 items （0.67 % ）, while in the control with 49 （ 8. 16% ）, the differences between the two groups was statistical significant （ P 〈 0. 01 ）. Conclusion The implementation of trying a good posture before the operation can improve the experience of patients un- dergoing general anesthesia in a lateral decubitus position, and be benefit for the implementation of individualized nursing in the surgery room.
