探究腹腔镜下辅助全胃切除及脏器联合切除治疗胃癌的临床效果及安全性

Investigation of clinical effect and safety by laparoscope-assisted total gastrectomy and combined evisceration in the treatment of gastric cancer

目的探究并分析腹腔镜下辅助全胃切除及脏器联合切除治疗胃癌的临床效果及安全性。方法回顾性分析30例进展期胃癌患者的临床资料,该组患者均采用了腹腔镜下辅助全胃切除及脏器联合切除治疗,观察其治疗效果。结果该组患者平均手术时间为(254.34±41.89)min,平均术中出血量为(375.45±23.78)ml,平均住院时间为(13.23±2.45)d。术后发生肺部感染4例,占13.33%,胰瘘5例,占16.67%,经过对症治疗及通畅引流后均痊愈,该组患者中无一例发生腹腔出血或吻合口瘘及肿瘤转移或死亡。结论采用腔镜下辅助全胃切除及脏器联合切除治疗胃癌的临床效果显著,安全性较高,值得推广。

Objective To investigate and analyze clinical effect and safety by laparoscope-assisted total gastrectomy and combined evisceration in the treatment of gastric cancer. Methods Clinical data of 30 patients with advanced gastric cancer were retrosepctively analyzed. All patients received laparoscope-assisted total gastrectomy and combined evisceration for treatment, and their curative effects were observed. Results The average operation time was（254.34±41.89） min, average intraoperative bleeding volume was（375.45±23.78） ml, and average hospital stay time was（13.23±2.45） d. There were 4 cases with postoperative pulmonary infection, accounting for 13.33%, and 5 cases with pancreatic fistula, accounting for 16.67%. All cases were cured after symptomatic treatment and unobstructed drainage, without intra-abdominal hemorrhage, anastomotic fistula, tumor metastasis and death. Conclusion Implement of laparoscope-assisted total gastrectomy and combined evisceration provides excellent clinical effect in treating gastric cancer, with high safety. It is worth promoting.
