巨细胞病毒感染相关肾损伤的临床分析

Clinical analysis of human cytomegalovirus infection associated renal injury

目的总结分析小儿人巨细胞病毒(HCMV)感染引起的肾脏损伤的临床特点。方法回顾性分析2013年1月至2014年12月期间326例在宝鸡市妇幼保健院住院诊断为HCMV感染患儿的临床资料,分析其肾脏损伤的发生情况,感染严重程度及HCMV复制水平与肾脏损伤之间的关系。结果 326例中肾脏损伤92例(28.22%),其中单纯性镜下血尿32例(9.82%)单纯性蛋白尿51例(15.64%),血尿+蛋白尿9例(2.53%)没有发现肉眼血尿及急性肾损伤者,不同年龄、临床表现、HCMV复制水平与肾脏损伤之间差异均无统计学意义(均P>0.05)。结论巨细胞病毒感染引起肾脏损伤的发生率较高,以单纯性蛋白尿居多。

Objective To analyze and summarize the clinical characteristics of children with renal injury caused by human cytomegalovirus （HCMV） infection. Methods A retrospective analysis was conducted on clinical data of 326 children who were diagnosed with HCMV infection in Matemal and Child Health Hospital of Baoji during the period of January 2013 to December 2014, and the incidence of renal injury, severity of infection, and relationship between HCMV replication level and renal injury were analyzed. Results In 326 cases, there were 92 cases （28.22%） with renal injury, including 32 cases （9.82%） of simple microscopic hematuria, 51 cases （ 15.64% ） of simple proteinuria and 9 cases （2.53%） of proteinuria with hematuria. No case was found with gross hematuria or acute renal injury. The differences in age, clinical manifestations, HCMV replication level and renal injury were not statistically significant （ all P 〉 0.05 ）. Conclusion The incidence of renal injury caused by HCMV infection is relatively high, and simple proteinuria is dominated.
