摘要
目的观察伴有眼底病变的妊娠高血压疾病患者随访中血压及视力变化情况。方法 (1)收集伴有眼底病变的妊娠高血压疾病患者98例,行眼部检查包括矫正视力,裂隙灯检查,眼底检查,并随访6个月,观察患者血压及视力变化情况及其相互关系。(2)随访观察的血压及矫正视力采用方差分析及pearson相关分析。结果 (1)随着随访时间变化,收缩压和舒张压呈下降趋势,双眼矫正视力均呈上升趋势,且二者变化均具有统计学意义(P<0.01)。(2)收缩压、舒张压和矫正视力之间存在高度正相关(相关系数分别为0.77、0.66、0.60、0.53),即在随访的过程中,随着血压的下降,视力呈上升趋势,两者之间存在相关性。结论血压的控制是妊娠高血压疾病患者眼底病变治疗的关键。孕期发现血压升高、视力下降应定期检查眼底,及时采取措施,以降低发生妊娠高血压疾病严重视网膜病变的风险。
OBJECTIVE To analyse variation of blood pressure and visual acuity in patients with fundus changes complicating hypertensive disorder of pregnancy during follow-up. METHODS 1. Ninety-eight cases of hypertensive disorder of pregnancy plus fundus changes were collected, all of whom were examined in terms of corrected visual acuity, slit lamp examination, fundus examination and they were followed for 6 months to observe the changes of blood pressure, visual acuity and the possible mutual relationships. 2. Variance analysis and Pearson correlation analysis were used for blood pressure and corrected visual acuity observed in follow-up. RESULTS 1. As time passed by, systolic and diastolic blood pressure decreased while binocular visual acuity showed an upward trend, and the change was statistically different(P〈0.01). 2. Systolic blood pressure, diastolic blood pressure and corrected visual acuity shared a high positive correlation(correlation coefficients were 0.77, 0.66, 0.60 and 0.53). In the course of follow-up, with the decline of blood pressure, vision showed an upward trend and there was a correlation between the two. CONCLUSIONS Management of blood pressure was the key to treatment of fundus diseases in patients with hypertensive disorders complicating pregnancy. During pregnancy, patients with blood pressure increase and visual acuity decrease history should be checked regularly and timely to reduce the risk of severe retinopathy of hypertensive disorders complicating pregnancy.
