尿白细胞、细菌及亚硝酸盐的联合检测在诊断尿路感染中的应用 被引量:21

摘要 目的 :探讨尿白细胞、细菌定量检测联合亚硝酸盐定性检测在诊断尿路感染(urinary tract infection,UTI)中的应用。方法:将551份患者中段尿标本做定量细菌培养,并采用UF-1000i尿沉渣分析仪和AX-4030尿干化学分析仪进行常规检测,记录亚硝酸盐(NIT)结果、白细胞(WBC)计数、细菌(BAC)计数,将上述结果同尿细菌培养结果进行比较。结果:NIT定性试验的灵敏度为38.3%,特异度为97.6%,阳性预测值为93.3%,阴性预测值为64.6%,准确度为70.1%。WBC计数和BAC计数试验的灵敏度、特异度、阳性预测值、阴性预测值、准确度分别为86.7%、49.8%、60.0%、81.2%、67.0%和65.2%、71.1%、66.8%、70.4%、68.8%。3项联合应用诊断UTI的灵敏度为91.4%,特异度为44.1%,阳性预测值为58.6%,阴性预测值为85.5%,准确度为66.1%。结论:WBC计数试验的灵敏度相对较高,可以较大程度筛选出尿路感染的患者,而BAC计数和NIT灵敏度比WBC计数低,但特异度和阳性预测值较高。3项联合应用时,可以最大程度地筛选出UTI患者,其较高的阴性预测值也可在很大程度上排除UTI。 Objective To investigate the application of quantitative measurement of urine white blood cell and bacteria combined with qualitative detection of urine nitrite in the diagnosis of urinary tract infection (UTI).Methods Totally 551 midstream urine samples underwent quantitative bacterial culture, and went through conventional detection with UF-1000i urinary sediment analyzer and AX-4030 urine dry-chemistry analyzer to measure urine nitrite (NIT), white blood cell (WBC) and bacteria (BAC). The results were compared with those by urine culture. Results The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of NIT qualitative detection were 38.3%, 97.6%, 93.3(/0, 64.6% and 70.1%, and the values of WBC and BAC were respectively 86.7% and 65.2%, 49.8% and 71.1%, 60.0% and 66.8%, 81.2% and 70.4%, and 67.0% and 68.8%. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy in case of combined application of NIT, WBC and BAC were 91.4%, 44.1%, 58.6%, 85.5% and 66.1% respectively.Conclusion WBC count behaves better than BAC count and NIT detection in the sensitivity for diagnosing UTI, while the latter two ones have high values in the specificity and positive predictive value. Combined application of NIT, WBC and BAC measurement facilitate to diagnose UTI, and can exclude UTI in case of high negative predictive value.
