单侧肾上腺切除术治疗双侧肾上腺大结节增生疗效分析 被引量:5

摘要 目的:探讨单侧肾上腺切除术治疗双侧肾上腺大结节增生(AIMAH)的可行性。方法:回顾性分析确诊为AIMAH的19例患者(男性11例,女性8例)的临床表现、实验室及影像学检查、病理结果、治疗及预后。结果:内分泌检查血浆皮质醇昼夜节律变化消失,24 h尿游离皮质醇升高,促肾上腺皮质激素释放激素(ACTH)受抑制或正常,小剂量地塞米松抑制试验均不受抑制,8例大剂量地塞米松抑制试验受到抑制。18例行单侧肾上腺切除术治疗,病理示肾上腺皮质结节性增生。10例临床型库欣综合征患者单侧肾上腺切除术后3~6个月库欣综合征消失,平均随访3年,库欣综合征无复发。另外2例临床型库欣综合征患者术后复发分别予对侧肾上腺全切除术、次全切除术,术后库欣症状缓解,给予糖皮质激素替代治疗。6例亚临床库欣综合征患者,术后高血压高血糖均有改善。结论:单侧肾上腺切除术治疗双侧肾上腺大结节增生是有效安全的手段。
