清肺化浊祛瘀汤治疗急性加重期慢性阻塞性肺疾病50例

Qingfei Huazhuo Quyu Treating 50 Chronic Obstructive Pulmonary Disease Patients During Acute Exacerbation

目的探讨清肺化浊祛瘀汤治疗急性加重期慢性阻塞性肺疾病(AECOPD)的临床疗效。方法 100例AECOPD患者随机分为治疗组和对照组各50例。对照组给予控制性氧疗,抗感染、激素、支气管舒张剂等常规西医治疗。在接受对照组治疗的基础上,治疗组加用自制清肺化浊祛瘀汤,1剂/d,7d为一个疗程,两组观察疗程均为2周。治疗前后,对主要症状进行评分;采用《呼吸功能状态和呼吸困难问卷修改版》(PFSDQ-M)量表进行生存质量评价;进行肺功能评价。结果经Ridit分析,治疗组临床疗效优于对照组,差异有统计学意义(P﹤0.05)。治疗后治疗组症状体征和生存质量评分均低于对照组(P﹤0.01)。治疗后对照组呼气容积(FEV1)和呼气容积占用力肺活量(FEV1 FVC)有所改善,但差异无统计学意义;治疗组治疗后FEV1和FEV1/FVC较治疗前有所增高,并高于对照组(P﹤0.01)。结论清肺化浊祛瘀汤能减轻AECOPD临床症状、改善肺功能,提升患者的生命质量,提高临床治愈率。

Objective To investigate the effect of Qingfei Huazhuo Stasis Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） clinical curative effect. Methods 100 cases of patients with AECOPD were randomly divided into into the treatment group and control group with 50 cases in each. The patients in the control group were treated by controlled oxygen therapy, anti infection, hormone, bronchodilator routine treatment of Western medicine. On the basic treatment of control group, the treatment group was treated with self-made Huazhuo Qingfei Quyu decoction, daily 1 agent, 7 days for a course of treatment. The course of treatment in the two groups are 2 weeks. Before and after treatment, scores of main symptoms of respiratory function; using the〈 Respiratory function status and Dyspnea Questionnaire Revised（PFSDQ-M） 〉scale to evaluate the quality of life; The pulmonary function was appraised before and after treatment. Results using Ridit analysis were done, cure Treatment of the clinical curative effect is better than that of the control group, and the difference is statistically significant（P〈0.05）. After the treatment, the treatment group signs and symptoms and quality of life scores were lower than the control group（P〈0.01）. After treatment in the control group expiratory volume（FEV1） and expiratory volume accounted for FVC（FEV1/FVC） has improved, but the difference was not statistically significant; FEV1 and FEV1/FVC in the treatment group after treatment than before treatment were increased, and higher than control group（P〈0.01）. conclusion Qingfei Huazhuo stasis soup can relieve clinical symptoms of patients with acute exacerbation of chronic obstructive pulmonary disease, improve lung function, improve the quality of life of patients, improve the clinical cure rate.
