摘要
目的探讨青年人群臂踝动脉脉搏波传导速度(baPWV)、颈动脉内膜中层厚度(IMT)和血流介导的血管舒张功能(FMD)的相关性。方法对陕西汉中老君乡10个自然村人群心血管病危险因素进行调查,选取其中完成IMT、FMD、baPWV 3项检查,符合入排标准的30~45岁青年人群473名进行体格及生化指标检查。BP-203RPEⅢ动脉硬化检测仪测量baPWV,超声检测IMT和FMD。应用Pearson相关和多元线性回归分析baPWV与IMT、FMD的相关性。结果与男性相比,女性FMD值较高[(10.28±4.25)%比(9.38±4.40)%,P〈0.05],baPWV值较低[(12.26±2.05)m/s比(13.59±1.96)m/s,P〈0.01],两者IMT水平差异无统计学意义。Pearson相关分析结果显示,IMT与FMD、baPWV的相关性无统计学意义,FMD和baPWV则呈负相关(r=-0.35,P〈0.01)。多元线性回归分析校正性别、年龄、收缩压、舒张压、体质量指数、腰臀比、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、空腹血糖、血肌酐、血尿酸等心血管病相关因素后,发现FMD是baPWV的影响因素(B=-8.689,P〈0.01)。结论在本组青年人群中,baPWV和FMD呈负相关,而IMT与baPWV及FMD均无相关性。
Objective To investigate the correlation among brachial-ankle pulse wave velocity(baPWV),carotid artery intima-media thickness(IMT)and flow-mediated dilation(FMD)in young adults. Methods Cardiovascular disease risk factors were investigated among the subjects from 10 villages of Laojun Hanzhong,Shaanxi. Finally,473 participants aged between 30 to 45 years old,who performed the examinations of IMT,FMD and baPWV and met the inclusion and exclusion criteria were included. BaPWV was detected with arteriosclerosis detector BP-203RPEⅢ. IMT and FMD were measured by ultrasound. Pearson correlation analysis and multiple linear regression analysis were used to analyze the correlation among IMT,FMD and baPWV. Results Compared with males,the FMD level was higher[(10.28±4.25)% vs(9.38±4.40)%,P0.05],while baPWV level was lower[(12.26±2.05)vs(13.59±1.96)m/s,P0.01]in females. The difference of IMT level between males and females was not statistically significant. No significant correlation was found between IMT and baPWV or FMD by Pearson correlation analysis. However,FMD was negatively correlated to baPWV(r=-0.35,P0.01). Multiple linear regression analysis indicated that FMD was an influencing factor of baPWV after adjusted for the related factors of cardiovascular disease,such as gender,age,systolic blood pressure,diastolic blood pressure,body mass index,waist-hip ratio,triglycerides,total cholesterol,low-density lipoprotein cholesterol,high density lipoprotein cholesterol,fasting plasma glucose,serum creatinine and uric acid(B=-8.689,P0.01). Conclusion In young adults,FMD is negatively correlated to baPWV. However,no significant correlation is found between IMT and baPWV or FMD.
