摘要
目的:探讨中国北方汉族人群中带状疱疹后神经痛(postherpetic neuralgia,PHN)的发病与人类白细胞抗原(human leukocyte antigen,HLA)的相关性。方法:收集2013年12月至2015年1月就诊于中国医科大学附属第一医院疼痛科PHN患者42例,及该院体检中心100例健康人血样,采用直接测序分型(PCR-SBT)的方法对142例样本进行HLA-A等位基因检测,并比较其分布频率及差异与PHN发病的相关性。结果:本研究中健康人群组中共检出17种HLA-A等位基因型,符合Hardy-Weinberg遗传平衡定律。PHN患者中的HLA-A*2601、-A*3004、-A*3303的基因频率明显高于健康人组(P<0.05)。而PHN患者中HLA-A*0201的基因频率明显低于健康人组(P<0.05)。结论:HLA-A*2601、-A*3004、-A*3303、-A*0201与中国北方汉族人群PHN发病相关,其中HLAA*2601、-A*3004、-A*3303为PHN发病的易感基因,而HLA-A*0201为其保护性基因。
Objective: To observe the association between the incidence of postherpetic neuralgia(PHN) and human leukocyte antigen(HLA) in the Han population in northern China. Methods: Collecting 42 patients suffered from PHN, who received treatment in the department of pain medicine of the First Affiliated Hospital of China Medical University, from December 2013 to January 2015, and 100 healthy persons who visited to Physical examination center. HLA-A, B and DRB1 genotypes were detected by direct sequencing typing(PCR-SBT) in 142 samples, the frequencies of alleles were calculated, and the association between gene polymorphism and the incidence of PHN was compared. Results: In this study, 17 kinds of HLA-A genotypes were detected. Calculating by chi square test, the distribution conformed to Hardy-Weinberg equilibrium. The frequencies of HLA-A*2601,-A*3004 and-A*3303 gene in PHN patients were significantly higher than that in the controls, and the frequency of HLA-A*0201 was lower than that in the controls. Conclusion: HLA-A*2601,-A*3004,-A*3303 and-A*0201 alleles were demonstrated with closed association with PHN. HLA-A*2601,-A*3004,-A*3303 are susceptibility alleles of PHN, and HLA-A*0201 is protective gene of PHN.
