PPH与外剥内扎术治疗混合痔疗效对比观察

Comparison on the Effects of PPH and External Dissection ＆ Internal Ligation in the Treatment of Mixed Hemorrhoids

为了探讨吻合器痔上黏膜环切术（PPH）与传统外剥内扎术治疗混合痔的临床疗效，回顾分析我院2008年5月至2010年8月行手术治疗的238例混合痔患者的临床资料。对比分析治疗组（119例）采用吻合器痔上黏膜环切术与对照组（119例）采用混合痔外剥内扎术的差异。观察2组患者术后一般情况、并发症及近远期疗效。结果显示，2组近期疗效无明显差异，而在手术时间、出血量、疼痛、水肿、康复时间、远期复发等方面差异具有统计学意义（P〈0．05）。结果表明，与传统外剥内扎术相比，PPH手术时间短、术后并发症少，是治疗混合痔近期疗效优越的一种微创手术，但其远期复发率较高。

This study was to compared the clinical effect of PPH for mixed hemorrhoids and external dissection ＆ internal ligation for mixed hemorrhoids, retrospectively analyzed the data of 238 cases who had received surgery for their mixed hemorrhoids in author＇s hospital from May 2008 to August 2010 yr., and compared surgery difference between treatment group（119 cases） recieved PPH and control one （119 cases） recieved mixed hemorrhoids external dissection ＆ internal ligation in the following items as general status,complication, as well short-term and long-term effects after operation. As results, in short-term effect there was no statistical difference（ P 〉0.05） ,but in operative time,bleeding volume,pain,edema,rehabilitation time, and long-term recurrence, etc there was statistical difference（ P 〈0.05）. Results show that compared with routine external dissection ＆internal ligation PPH has such merits as short operative time and less postoperative complication,is a mini-invasive procedure with excellent short-term effect for mixed hemorrhoids,but its long-term recurrence rate is higher.
