匹多莫德联合氯雷他定治疗小儿过敏性鼻炎疗效观察

Observation of curative effect of pidotimod combined with clarityne on paediatric allergic rhinitis

目的观察匹多莫德联合氯雷他定治疗小儿过敏性鼻炎的临床疗效及对外周血Th1/Th2细胞因子表达的影响。方法将100例过敏性鼻炎患儿随机分为观察组和对照组各50例,2组均给予氯雷他定口服,观察组在此基础上给予匹多莫德口服,观察2组治疗前及治疗4周后临床症状及体征评分和血清白细胞介素-4(IL-4)、IL-5、IL-12、肿瘤坏死因子α(TNF-α)水平,统计2组症状体征缓解时间、临床疗效及不良反应发生情况。结果 2组治疗4周后临床症状及体征评分均明显低于治疗前(P均<0.05),且观察组各项评分均明显低于对照组(P均<0.05);2组治疗4周后血清IL-4、IL-5水平均明显低于治疗前(P均<0.05),血清IL-12、TNF-α水平均明显高于治疗前(P均<0.05),且治疗组各指标改善情况均明显优于对照组(P均<0.05);观察组鼻痒、流涕、鼻塞、打喷嚏症状缓解时间均明显短于对照组(P均<0.05);治疗4周后治疗组总有效率明显高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论在基础治疗基础上给予匹多莫德口服能进一步改善过敏性鼻炎患儿的Th1/Th2细胞因子失衡,并能提高治疗总有效率,缩短伴随症状消失时间,且不良反应少。

Objective It is to observe the clinical effect of pidotimod combined with clarityn on paediatric allergic rhinitis,and effect on the expression of Th1 / Th2 cytokines in serum. Methods 100 children with rhinallergosis were randomly divided into control group and observation group,each group had 50 cases. Both the groups were treated with clarityne by oral administration,and the observation group was added with pidotimod on this basis. The scores of clinical symptoms and signs,the serum levels of IL-4,IL-5,IL-12 and TNF-α before treatment and 4 weeks after treatment were observed in both groups,the relieving time of symptom and sign,clinical curative effect and side effect were compared between the two groups. Results In 4 weeks after treatment,the scores of clinical symtom and sign were significantly lower than that before treatment in both groups（ P〈0. 05）,and the decreases in treatment group were more significant compared with the control group（ P〈0. 05）;The serum levels of IL-4,IL-5 were lower while that of IL-12 and TNF-α were higher than that before treatment in both groups,and the decrease or increase were more significant compared with the control group（ P〈0. 05）. The relieving time of symptoms such as rhinocnesmus,runny nose,rhinobyon,sneeze were shorter and the total effective rate was higher in observation group than that in control group（ P〈0. 05）,there was no significant difference in occurrence rate of side effect between the two groups（ P〉0. 05）. Conclusion The basial treatment combined with pidotimod can improve the disequilibrium of Th1 / Th2 cytokines and raise total effective rate,shorten recovery time of complicating symptoms with less side effect in the children with rhinallergosis.
