糖尿病足病的中西医治疗进展

The Treatment Progress of Integrated Medicine in the Treatment of Diabetic Foot Disease

我国可能已成为世界上糖尿病患病人数最多的国家。糖尿病足病是糖尿病最严重的和治疗费用最高的慢性并发症之一,重者可导致截肢甚至危及生命。糖尿病患者下肢截肢的相对风险是非糖尿病患者的40倍。近年的调查显示,我国三甲医院非创伤性截肢患者中约有三分之一为糖尿病所致。大约85%的截肢是由于足溃疡(DFU)引发的,15%左右的糖尿病患者会在其一生中发生DFU^([1])。世界卫生组织1999年将糖尿病足定义为糖尿病患者由于合并神经病变及各种不同程度的下肢血管病变而导致的下肢感染,溃疡形成和(或)深部组织的破坏^([2])。中医虽无糖尿病足的记载但本病归属于中医消渴的"脉痹"、"脱疽"范畴。预防和治疗糖尿病足已经成为非常重要的社会问题。为降低糖尿病足的发病率,控制糖尿病足的致残率和截肢率,提高患者生活质量,本文综合近年的文献资料,从糖尿病足患者的教育以及中西医治疗作一综述。

China may have become the largest number of people with diabetes in the world. Diabetic foot disease is the most serious and one of the most expensive chronic complications, and serious illness can lead to amputation and even life-threatening. The relatie risk of diabetic lower limb amputation is 40 times of diabetes patients. In recent years, the survey showed that China ＇s tertiary level1 st class patients with traumatic amputation of about a third of caused by diabetes. About 85 % of the amputation was caused by diabetic foot ulcers（DFU）, and about 15% of patients with diabetes may occur during their lifetimes DFU. The world health organiza-tion（WHO） defined diabetic foot diabetes in 1999 due to the merger of neuropathy and various degrees of lower limb vascular le-sions caused by lower limbs infection, ulcer formation and（or） destruction of deep tissue. The traditional Chinese medicine though no records of diabetic foot disease, it belongs to the category of ＂ vessel bi-disease＂ and ＂ gangrene of digit＂ of consumptive thirst in TCM. The prevention and treatment of diabetic foot has become a very important social problem. To reduce the incidence of dia-betic foot, control of diabetic foot morbidity and amputation rate, improve patient quality of life, in this paper, a synthesis of the liter-ature from education of traditional Chinese medicine and Western medicine in the treatment of diabetic foot was reviewed.
