摘要
背景目前临床研究证实了长时间机械通气(mechanicalventilation,MV)是术后谵妄(postoperativedel试um,POD)发作的危险因素。对通气参数和呼吸模式的不适反应,激发大脑皮质诱发电位,造成一系列的神经心理改变和谵妄发作。目的了解肺脑交互在MV致谵妄中的作用。内容综述MV诱发谵妄发作过程中的肺脑交互作用机制及其高危因素与防治。趋向通过对MV患者谵妄发作的病理生理机制的研究,完善MV危重患者的管理方案,减少中枢神经系统(centralnervesystem,CNS)并发症。
Background It has been demonstrated that long-time mechanical ventilation (MV) is one of the risk factors of postoperative delirium (POD). Cortical evoked potential is triggered when patients don't adapt to ventilation parameters and modes, which developed into a series of neuropsychological changes and the onset of delirium. Objective This review try to analysisthe role that lung-brain interaction played in the progress of MV trigger delirium. Content Review the mechanism of lung-brain interaction in the MV trigger delirium and the related risk factors and preventing strategies. Trend Through research the pathophysiology of delirium to improve the management of critically ill patients those received MV and to reduce the incidence of central nerve system(CNS) complications.
