综合护理干预在腹腔镜胆囊切除术后胆漏病人中的应用

Application of comprehensive nursing intervention in patients with bile leakage after laparoscopic cholecystectomy

[目的]探讨综合护理干预应用于腹腔镜胆囊切除术后病人中的效果。[方法]将60例腹腔镜下胆囊切除术后存在胆漏的病人按照随机数字表法分为观察组和对照组各30例,对照组给予常规护理,观察组在常规护理的基础上给予综合护理干预,比较两组病人术后48h引流量、胆漏停止时间、临床治疗效果(痊愈出院、再次手术治疗及临床死亡情况)及住院时间。[结果]观察组病人术后48h引流量基本正常,且显著多于对照组(P<0.05),胆漏停止时间显著快于对照组(P<0.05),痊愈出院率显著高于对照组(P<0.05),需要再次实施手术治疗的比例显著低于对照组(P<0.05),住院时间显著短于对照组(P<0.05)。[结论]对腹腔镜胆囊切除术后胆漏病人实施有效的综合护理干预能显著减少胆漏时间、缩短住院时间、改善病人预后。

Objective:To probe into the application effect of comprehensive nursing intervention for patients with bile leakage after laparoscopic cholecystectomy.Methods:A total of 60 cases with bile leakage after laparoscopic gallbladder were randomly divided into control group and observation group,30 cases in each.The patients in control group received the routine care,and the patients in observation group received the comprehensive nursing interventions based on the routine care.Then to compare the postoperative drainage volume,bile leakage stopping time,clinical treatment effect(recovery and discharge,re-operation treatment and clinical death)and the length of stay between both groups after 48-hour.Results:The drainage amount of patients in observation group was basically normal,and was significantly higher than that in control group(P<0.05).Bile leakage stopping time of patients in observation group was significantly faster than that in control group(P<0.05).The cured discharge rate of patients in observation group was significantly higher than that in control group(P<0.05).The proportion of surgical treatment of patients in observation group was significantly lower than that in control group(P<0.05).Hospital stay of patients in observation group was significantly shorter than that in control group(P<0.05).Conclusion:Comprehensive nursing intervention could significantly reduce the time of bile leakage,shorten the length of stay and improve the prognosis of patients with bile leakage after laparoscopic cholecystectomy.
