吸烟与帕金森病患病风险的Meta分析

Meta-analysis of risk between smoking and Parkinson's disease

目的探讨吸烟与帕金森病发病之间的关系。方法通过中国生物医学文献数据库、万方数据库、维普数据库、中国知网、Pub Med等收集2016年1月之前发表的关于吸烟与帕金森病的相关文献。用帕金森、危险因素、吸烟为检索词自由组合,并用逻辑连接词＂AND＂或＂OR＂进行连接。运用Stata 11.0中Meta-analysis模块对文献进行统计分析。结果共纳入16篇文献（病例∶对照=6765∶309 454）。吸烟状态与帕金森病（PD）患病风险：现在吸烟者,OR=0.34,95%CI 0.22~0.45,P=0.000;过去吸烟者,OR=0.59,95%CI 0.52~0.65,P=0.001。吸烟年限与PD患病风险：〈20年,OR=0.65,95%CI 0.48~0.82,P=0.000;烟龄≥20年,OR=0.55,95%CI 0.46~0.65,P=0.000。吸烟量与PD患病风险：〈20包/年,OR=0.76,95%CI 0.66~0.86,P=0.086;≥20包/年,OR=0.45,95%CI：0.37~0.53,P=0.000。但是在吸烟量的分层分析中≥69岁以上人群,吸烟对PD患病风险无统计学意义（OR=0.85,95%CI 0.65~1.05,P=0.450）。结论吸烟对PD发生的影响有待进一步研究。

Objective To discuss the relationship between smoking on Parkinson＇s disease（PD）. Methods The references were retrieved through the databases of CBM, Wangfang, VIP, CNKI, Pub Med. A Meta-analysis was performed with Stata 11.0 on the relationship between smoking and Parkinson＇s disease. Words of PD, risk factors, smoking were searched. Results Sixteen case-control papers（PD cases∶control cases： 6765∶309 454） as were included. The results between smoking status and the risk for PD were： current smokers, OR = 0.34, 95%CI 0.22-0.45, P = 0.000; ex-smoker,OR = 0.59, 95%CI 0.52-0.65, P = 0.001. The results between smoking years and the risk for Parkinson＇s disease were：20 years, OR = 0.65, 95%CI 0.48-0.82, P = 0.000; ≥20 years, OR = 0.55, 95%CI 0.46-0.65, P = 0.000. The results between packs/year and the risk for PD were： 20 packs/year, OR = 0.76, 95%CI 0.66-0.86, P = 0.086; ≥20 packs/year, OR = 0.45, 95% CI 0.37-0.53, P = 0.000. However, the analysis obtained packs/year（≥69 old） did not show correlation, OR = 0.85, 95%CI 0.65-1.05, P = 0.450. Conclusion Further studies are needed to study the association between smoking and PD.
