高血压患者肾动脉阻力指数与肾功能的相关性分析

Hypertensive patients with renal artery resistance index and renal function correlation analysis

目的探讨高血压患者肾动脉阻力指数与肾功能的相关性。方法选取2014年6月~2015年7月我院收治的50例高血压患者作为研究对象,分为Ⅰ级、Ⅱ级、Ⅲ级高血压组,选择同期无高血压病史的50例作为对照组,两组都采用彩色多普勒超声仪器检测肾动脉阻力指数,同时检测尿素氮、尿β2微球蛋白、血清肌酐、肌酐清除率。结果Ⅱ级和Ⅲ级高血压患者肾动脉阻力指数明显高于对照组,差异有统计学意义(P<0.05);Ⅲ级高血压患者的尿素氮(8.17±3.01)mmol/L、尿β2微球蛋白(0.85±0.11)mg/L、血清肌酐(81.26±9.02)μmol/L以及肌酐清除率(74.76±9.03)ml/(min·1.73m^2)与对照组比较,差异有统计学意义(P<0.05),并且高血压患者肾动脉指数和尿素氮、尿β2微球蛋白、血清肌酐呈正相关,与肌酐清除率呈负相关。结论肾动脉阻力指数是评价高血压患者早期肾损伤的一个非常重要的指标。

Objective To explore the renal artery resistance index in patients with hypertension and renal function correlation. Methods From June 2014 to July 2015 in our hospital 50 cases of hypertensive patients as research sub- jects,divided into Ⅰ、Ⅱ、Ⅲ hypertensive group,compared with no history of hypertension select 50 cases as a control group,two groups of patients are used color Doppler ultrasound equipment to detect renal artery resistance index,but al- so need to detect urea nitrogen,urinary β2-microglobulin, serum ereatinine,creatinine clearance. Results Ⅱ and Ⅲ renal artery resistance index in patients with hypertension was significantly higher,with statistically significant （P〈0.05）. Grade m hypertensive patients urea nitrogen （8.17±3.01） mmol/L,urinary β2-microglobulin （0.85±0.11） mg/L,serum creatinine （81.26±9.02） μmol/L and creatinine clearance （74.76±9.03） ml/（min·1.73m2）compared with the control group significantly（P〈0.05）,and hypertensive patients with renal artery index and blood urea nitrogen,urinary β2-microglobulin, serum creatinine showed positive correlation with creatinine clearance showed negative correlation. Conclusion Evalua- tion of renal artery resistance index is a very important indicator of early kidney damage in patients with hypertension.
