血清25羟基维生素D与膝关节骨性关节炎疼痛相关性研究 被引量:1

摘要 目的 探讨血清25羟基维生素D水平与膝关节骨性关节炎疼痛之间的关系。方法 选取2012年8月-2014年10月我院门诊就诊的膝关节骨性关节患者177例为观察对象,分为实验组和对照组两组。实验组就诊时以疼痛为主诉患者,共135例,对照组就诊时以非疼痛为主诉,而为定期复查开药患者,共42例。所有患者入组时行膝关节负重正位X线,采用Kellgren和Lawrence方法对骨性关节炎严重程度进行分级,并行空腹检测血清25羟基维生素D水平。应用SPSS 13.0软件对两组患者25羟基维生素D水平进行相关性分析。结果 实验组患者25羟基维生素D为(14.893±7.570)ng/ml,对照组为(19.870±8.460)ng/ml,两组之间差异有统计学意义(t=5.875,P〈0.01)。通过负重正位X线,实验组患者按照Kellgren和Lawrence标准分级,1级患者血清25羟基维生素D为(11.103±2.765)ng/ml,2级患者为(15.074±7.952)ng/ml,3级为(15.176±5.351)ng/ml,4级为(15.908±6.789)ng/ml,各亚组之间差异无统计学意义(F值=0.886,P=0.321)。实验组患者25羟基维生素D值水平与疼痛之间呈负相关(r=0.801,P〈0.05)。结论 血清25羟基维生素D水平可能是膝关节骨性关节炎疼痛的一个独立危险因素,但由于本实验样本量小,需进一步实验研究。 Objective To investigate the relationship between 25-hydroxyvitamin D and pain in knee osteoarthritis pa- tients. Methods 177 cases of in knee osteoarthritis outpatients were divided into experimental group and control group from August 2012 to October 2014 in Aviation General Hospital.135 cases were divided into experimental group with chief complaint of knee pain,42 cases were divided into control group with chief complaint of prescription without knee pain.The severity of osteoarthritis was graded with the standard Kellgren, and Lawrence throw weight-bearing antero- posterior X-ray.All the patients underwent fasting serum levels of 25-hydroxyvitamin D testing.SPSS 13.0 software was applied for correlation analysis. Results The concentration of serum 25-hydroxyvitamin D was respectively (14.893+ 7.570)ng/ml and (19.870±8.460)ng/ml in experimental group and control group,the difference was statistically significant (t=5.875,P〈0.01).The result of the graded measurement showed that the 25-hydroxyvitamin D level of osteoarthritis pa- tients with 1 K/L was (11.103±2.765)ng/ml,2 K/L was (15.074±7.952)ng/ml,3 K/L was (15.176±5.351)ng/ml and 4 K/L was (15.908±6.789)ng/ml,there were no statistical difference between the four subgroups (F=0.886,P=0.321).The level of 25-hydroxy vitamin D and pain values were negatively correlated in experimental group (r=0.801, P〈0.05). Conclusion The present study indicated that the serum level of 25-hydroxyvitamin D may be associated with pain in knee os- teoarthritis patients,further replication of our result with more samples will be required to study the relationship be- tween 25-hydroxyvitamin D and knee pain.
