腹腔镜卵巢囊肿剥除术中不同止血方式对卵巢女性激素水平的影响

Influence of different hemostatic methods on female ovarian hormone levels during laparoscopic ovarian cyst surgery

目的 探讨腹腔镜卵巢囊肿剥除术中不同止血方式对卵巢女性激素水平的影响。方法 选取2013年1月-2015年1月在本院就诊的60例卵巢囊肿患者作为研究对象,依据止血方式的不同分为缝合组和电凝组,各30例。缝合组采用镜下缝合止血,电凝组采用双极电凝止血。比较两组的手术时间、术中出血量、并发症发生率及手术前后的促卵泡激素、17β-雌二醇及促黄体生成素水平。结果 缝合组的手术时间显著长于电凝组,术中出血量显著多于电凝组,差异有统计学意义（P〈0.05）。两组的并发症发生率比较,差异无统计学意义（P〉0.05）。缝合组术后5个月的促卵泡激素及促黄体生成素水平显著低于电凝组,17β-雌二醇水平显著高于电凝组,差异有统计学意义（P〈0.05）。结论 缝合止血技术对患者卵巢女性激素水平的影响较小,但手术时间长、术中出血量多。

Objective To explore the influence of different hemostatic methods on female ovarian hormone levels during laparoscopic ovarian cyst surgery. Methods 60 patients with ovarian cyst from January 2013 to January 2015 in our hospital were selected and divided into the suture group and the electrocoagulation group according to different hemo- static methods,30 cases in each group.The suture group was given suture hemostasis under the microscope,the electro- coagulation group was given bipolar coagulation hemostasis.Operation time,intraoperative bleeding volume,the complica- tion occurrence rate and the level of follicle-stimulating hormone （FSH）,17β-estradiol,luteinizing hormone （LH） before and after surgery in the two groups was compared. Results Operation time in the suture group was longer than that in the eleetrocoagulation group,intraoperative amount of bleeding in the suture group was more than that in the electroco- agulation group,with significant difference （P〈0.05）.There was no significant difference in the complication occurrence rate between two groups （P〉0.05）.The level of FSH,LH after surgery at 5 months in the suture group was lower than that in the electrocoagulation group,the level of 17β-estradiol in the suture group was higher than that in the electroco- agulation group,with significant difference （P〈0.05）. Conclusion Suture hemostasis has few influence on female ovarian hormones,but with operation time is long and large blood loss during surgery.
