《特殊情况紧急抢救输血推荐方案》在临床输血实践中的应用

Application of the recommended scheme for emergency blood transfusion under special circumstances in clinical transfusion practice

目的评价、推广《特殊情况紧急抢救输血推荐方案》(简称《推荐方案》)在临床输血中的应用。方法遵循《推荐方案》的具体指导内容及流程,对近2年交叉配血不合或/和抗体筛查阳性、ABO同型血液储备无法满足需求、ABO疑难血型、RhD阴性的患者实施紧急抢救输血的病例,分析评价其应用效果。结果成功对48例急危重症患者施行紧急抢救输血,其中47例患者得到及时、有效救治。结论《推荐方案》为特殊情况紧急抢救输血指明了方向,规范了流程,使临床一线广大输血工作者在特殊情况紧急抢救输血中有章可循,具有实际操作指导意义,能帮助临床输血工作科学化、规范化规避医疗风险。

Objective To evaluate and promote the application of a recommended scheme for emergency blood transfusion under special circumstances（ referred to as ＂recommended scheme＂） in clinical blood transfusion. Methods A recommended scheme was implemented in patients who have undergone emergency blood transfusion in recent two years,have experienced crossmatching incompatibility,were tested positive in antibody screening,had difficult ABO blood types,were Rh D negative,and / or encountered challenges to obtain same ABO blood type. The impact of the application was then analyzed and evaluated. Results Emergency resuscitation transfusion was successfully implemented for 48 patients with acute critical illness,in which 47 cases were timely and effective treatment. Conclusion The recommended scheme for emergency blood transfusion under special circumstances confers great practical significance,can help the rational use of clinical blood transfusion,and can be standardized to avoid medical risks. It is worth promotion and application.
