社区老年高血压病人直立性低血压发病率分析

Analysis of orthostatic hypotension morbidity in aged patients with hypertension from communities

目的：分析社区老年高血压病人直立性低血压（OH）的发病率。方法：对辖区内897例社区老年高血压病人开展体检,结果发现有OH病倒238例（26.5%）。根据年龄高血压患者被分为60-79岁组（727例）和80岁以上组（170例）;再根据体检的立位血压,每组分为OH组和非OH组两个亚组。分析各组临床一般资料及OH发病情况。结果：（1）两个年龄段男性与女性OH比例差异无统计学意义（P=415）;（2）80岁以上者直立性低血压的发病率显著高于60-79岁者（44.11%比22.42%）,P〈0.01;（3）于社区老年高血压80岁以上组,与卧位比较,立位0min、2min时,收缩压[（132.89±22.77）mmHg比（104.00±19.42）mmHg、（113.43±20.52）mmHg]及舒张压[（80.06±12.37）mmHg比（68.02±11.10）mmHg、（71.34±13.37）mmHg]均明显下降（P〈0.05或〈0.01）结论：目前社区老年高血压病人中直立性低血压的发病率较高,其中高龄老年人直立性低血压的发病率更高。

Objective： To analyze morbidity of orthostatic hypotension （OH） in aged patients with hypertension fromcommunities. Methods： A total of 897 aged patients with hypertension from communities received physical examination, in which there were OH patients of 238 cases （26. 5% ） . According to their age, patients-79 years group （n = 727） and 〉80 years group （n=170）. According to orthostatic hypertension blood pressure re-sults of physical examination,each group was further divided into OH subgroup and non-OH subgroup . General data and OH morbidity were analyzed and compared among all groups . Results： （1） There were no significant difference inOHmorbiditybetweenmenandwomen in60-79yearsgroupand 〉 8 0 years group,P = 0.4 1 5 ;⑵ Compared with60？79yearsgroup,therewas significant rise in in cidencerate of OH （22. 42% vs. 44. 11% ） in 〉 80 years group,P〈0. 01 ; （3） In〉80 years group,compared with recumbent position,there were significant reductions insystolic blood pressure [ （132. 89 ± 22. 77） mmHg vs. （104. 00 ± 19. 42） mmHg,（ 113. 43 ± 20. 52） mmHg] and diastolic blood pressure [ （80. 06 ± 12. 37） mmHg vs. （68. 02 ± 11. 10） mmHg,（ 71. 34 ± 13. 37） mmHg] inorthostatic position 0 min and 2 min,P 〈 0 . 05 or 〈 0 . 01. Conclusion： Current morbidity of OH is high in aged patients withpertension from communities,and t s higher in advanced aged people.
