微创和标准通道经皮肾镜取石术治疗复杂性肾结石的疗效比较

Comparison of Minimally Invasive and Standard Channel Percutaneous Lithotripsy in the Treatment of Complex Renal Calculi

目的比较分析微创（MPCNL）和标准通道经皮肾镜取石术（PCNL）治疗复杂性肾结石的疗效。方法选择2014年5月-2015年7月在我院进行治疗的60例复杂性肾结石患者为研究对象,将其随机分为观察组和对照组,每组各30例患者,对照组患者采用PCNL治疗,观察组患者采用MPCNL治疗,对比两组疗效。结果观察组结石清除率为93.3%（28/30）,与对照组的90.0%（27/30）比较差异无统计学意义,两组术后并发症发生率差异无统计学意义,观察组术中出血量低于对照组,对照组手术时间短于观察组,差异具有统计学意义（P〈0.05）。结论 MPCNL和PCNL治疗复杂性肾结石的疗效均良好,PCNL手术时间少,MPCNL术中出血量低,具体术式需要依据患者具体情况合理选择。

Objective To compare and analysis of MPCNL and PCNL of curative effect treatment the of kidney stones. Methods 60 patients with complex kidney stones were chosen as the research object from May 2014 to July 2015, its were randomly divided into observation group and control group, each group 30 cases,control group were treated by PCNL, the observation group were treated by MPCNL, compared two groups curative effect. Results Observation group, the stone clearance rate was 93.3%（28 / 30） and the control group 90.0%（27 / 30） and the difference no statistically significant. No statistically significant differences of postoperative complications after operation in the two groups. Observation group in blood loss is lower than the control group, control group, operation time shorter than the observation group, the difference has statistical significance（P〈0.05）. Conclusion The efficacy of MPCNL and PCNL in the treatment of complex kidney stones are good, PCNL operation time is little, the amount of bleeding in MPCNL operation is low, the specific operation type needs reasonable choice according to the specific situation of the patients.
