2012-2014年菏泽市15岁以下急性脊髓炎病例监测

Surveillance on Myelitis Cases Under Age of 15 in Heze City,2012-2014

目的对菏泽市2012-2014年15岁以下急性脊髓炎病例监测情况进行分析研究,初步评价菏泽市15岁以下脊髓炎病例报告监测和临床诊治工作进展,掌握其发病流行状况及规律,为保持无脊灰状态工作和脊髓炎病例诊治提供科学依据。方法根据急性弛缓性麻痹病例网络直报系统资料、流行病学个案调查资料、临床诊断资料和实验室病毒检测结果进行分析研究。结果菏泽市2012-2014年共报告15岁以下脊髓炎病例20例。2012-2014年15岁以下脊髓炎病例年平均报告发病率为0.38/10万。20例病例分布在全市9县区的20个乡镇(办事处)的20个行政村(社区),高度散发。48 h及时调查率为100.00%,合格大便标本采集率为100.00%。未检测到脊灰野病毒,1例检测到II型脊灰疫苗病毒,脊灰疫苗病毒分离率为5.00%,未检测到脊灰疫苗衍生病毒与疫苗高变异病毒。非脊灰肠道病毒分离率5.00%。结论菏泽市15岁以下脊髓炎病例报告监测和诊治工作取得了很大进展,但仍有不足之处。建议仍应加强15岁以下脊髓炎病例报告监测和诊治工作。

Objective To analyze the surveillance status of acute myelitis cases under the age of 15 during 2012 to2014 in Heze city,and to preliminarily evaluate the work progress of case reporting and clinical diagnosis and treatment,as well as to and to master the epidemic status and law of myelitis,and thus to provide scientific basis for maintaining polio- free status and treatment of myelitis cases. Methods The analysis was conducted based on direct reporting network system of AFP cases,epidemiological survey,clinical diagnosis and laboratory virus separation results. Results A total of 20 myelitis cases under the age of 15 were reported during 2012 to2014 in Heze city,with an average incidence of 0. 38 /105. The 20 myelitis cases were distributed in 20 administrative villages of 20 towns among 9 counties in Heze city,showing a highly distributed state. The timely investigation rate in 48 h was 100. 00%,and the adequate collected rate of stool specimen was 100. 00%. No wild polio virus was isolated,so did the polio vaccine- derived virus and polio vaccine variation virus. One case of polio vaccine virus was isolated and the isolating rate of polio vaccine virus was 5. 00%. The isolating rate of non- polio enteroviruses was 5. 00%. Conclusion Work of case reports and clinical diagnosis and treatment of myelitis cases under the age of 15 of Heze city has made a great progress,but still has shortcomings. Case reporting and clinical diagnosis and treatment of myelitis cases under the age of 15 in Heze city should be strengthened.
