三维适形放疗联合同步吉非替尼化疗对肺癌患者细胞因子CSF、TNF、IL-2的影响

Effect of three dimensional conformal radiotherapy combined with synchronous chemotherapy on CSF,TNF and IL-2 in lung cancer patients

目的探讨研究三维适形放疗联合同步吉非替尼化疗对肺癌患者细胞因子集落刺激因子(CSF)、肿瘤坏死因子(TNF)、白细胞介素2(IL-2)的影响。方法随机选取80例肺癌患者,采用双盲法分为实验组和对照组,每组各40例。对照组采用常规放疗同步吉非替尼化疗进行治疗,实验组采用三维适形放疗同步吉非替尼化疗进行治疗。观察比较两组治疗效果、毒性反应、治疗前后细胞因子CSF、TNF、IL-2水平变化。结果实验组治疗总有效率较对照组显著升高,差异具有统计学意义(P=0.016),毒性反应发生率低于对照组,但差异无统计学意义(P=0.101);治疗前实验组和对照组细胞因子CSF、TNF和IL-2的水平相近,治疗后两组比较,实验组CSF和IL-2水平较对照组显著升高[(0.3582±0.1068 pg/ml)vs.(0.2581±0.0912 pg/ml)、(3.9686±0.6453 ng/ml)vs.(3.2103±0.5476 ng/ml)],而TNF水平较对照组显著降低[(1.5632±0.2513 ng/ml)vs.(1.7545±0.2632 ng/ml)],差异有统计学意义(P<0.01)。结论采用三维适形放疗联合同步吉非替尼化疗治疗肺癌效果显著,毒性反应少,可以明显改善细胞因子CSF、TNF和IL-2水平。

Objective To explore and study the effect of three dimensional conformal radiotherapy combined with synchronous chemotherapy on CSF,TNF and IL- 2 in lung cancer patients. Methods 80 patients with lung cancer were randomly divided into experimental group and control group,40 cases in each group. The control group were treated with conventional radiotherapy combined with synchronous chemotherapy.The experimental group were treated with three dimensional conformal radiotherapy combined with synchronous chemotherapy. The treatment effect,adverse reactions,treatment of cytokines CSF,TNF,IL- 2 level changes of the two groups were observed and compared. Results The total effective rate of the treatment group was significantly higher than that of control group（ P = 0. 016）. The toxic and side effect of the former was slightly lower than that of the latter（ P = 0. 101）. Before treatment,the levels of IL- 2,TNF and CSF in the experimental group and the control group were similar,and after treatment,the levels of CSF,TNF and IL- 2 were（ 0. 3582 ± 0. 1068） pg / ml,（ 1. 5632 ± 0. 2513） ng / ml,（ 3.9686 ± 0. 6453） ng / ml in the experimental group,the control group respectively（ 0. 2581 ± 0. 0912） pg / ml,（ 1. 7545 ± 0. 2632） ng / ml,（ 3.2103 ± 0. 5476） ng / ml. Therefore,the differences were statistically significant（ P = 0. 001）. Conclusion The effect of three dimensional conformal radiotherapy combined with synchronous chemotherapy is significant,and the toxic and side effects are few. At the same time the levels of CSF,TNF and IL- 2 are significantly improved.
