疗前FDG最大标准摄取值预测局部晚期食管癌放疗效果分析

摘要 目的:探讨疗前PET-CT FDG最大标准摄取值(SUVmax)在局部晚期食管癌放疗预后中的价值。方法:回顾性分析行放疗为主的局部晚期食管癌患者82例,治疗前皆行全身18F-FDG PET-CT扫描,分别分析食管原发灶SUVmax(SUVmax-Eso)、区域转移淋巴结SUVmax(SUVmax-Ln)与临床各因素和临床疗效的关系。结果:(1)全组SUVmax-Eso中位数为9.6(3.3-32.6),SUVmax-Ln为6.1(1.8-27.3)。SUVmax-Eso与T分期(r=0.265,P=0.008)、食管病变长度(r=0.296,P=0.005)以及SUVmax-Ln(r=0.173,P=0.021)均相关。(2)局部复发与无复发患者的SUVmax-Eso中位数分别为10.3和8.6(U=260.30,P=0.037),SUVmax-Ln的中位数分别为7.3和5.9(U=568.90,P=0.812);远处转移与无转移患者SUVmax-Eso中位数分别为11.4和7.9(U=220.40,P=0.016),SUVmax-Ln的中位数分别为7.1和5.8(U=557.80,P=0.723)。(3)与3年无进展生存和总生存相关的最佳截断点SUVmax-Eso皆为9.7。SUVmax-Eso≤9.7与〉9.7患者3年无进展生存率与总生存率分别为76.0%与50.2%(χ^2=17.21,P=0.000)和78.2%与51.8%(χ^2=16.35,P=0.000)。与3年无进展生存和总生存相关的最佳截断点SUVmax-Ln均为7.6。SUVmax-Ln≤7.6与〉7.6患者3年无进展生存率与总生存率分别为77.6%与51.8%(χ^2=18.58,P=0.000)和79.8%与50.2%(χ^2=16.95,P=0.000)。多因素分析显示患者3年PFS的预后因素有SUVmax-Eso(HR=7.26,P=0.012)、T分期(HR=5.38,P=0.026);3年OS的预后因素有SUVmax-Eso(HR=8.67,P=0.009)、SUVmax-Ln(HR=4.92,P=0.035)。结论:疗前SUVmax-Eso可能是预测食管癌放疗后局部复发、远处转移和生存的有效生物学指标。T分期可能与局部区域复发有关,SUVmax-Ln可能与总生存有关。
