不同手术方法治疗前列腺增生合并膀胱结石的对比研究

Comparison of different surgical methods for the treatment of benign prostatic hyperplasia with bladder stone

目的 研究不同手术方法治疗前列腺增生合并膀胱结石的临床疗效.方法 对本院收治的129例患者进行回顾性分析.按照手术方法分为观察组和对照组,观察组采用气压弹道碎石联合前列腺电切术,对照组采用手术取石方法.比较两种治疗方法的治疗效果,疼痛VAS评分以及并发症发生情况.结果 两组患者取石时间、手术时间、术中出血量、膀胱冲洗时间、导管置留时间和住院时间在两组之间的分布差异均存在统计学意义（P＜0.05）,观察组的取石时间和手术时间大于对照组,而观察组的术中出血量、膀胱冲洗时间、导管置留时间和住院时间均小于对照组.两组患者术后疼痛VAS评分平均得分间的差异有统计学意义（P＜0.05）,观察组的疼痛VAS评分要低于对照组.两组术后并发症发生情况比较,感染、血尿和痔疮发生在两组之间的分布有统计学意义（P＜0.05）,观察组并发症发生率要优于对照组.结论 气压弹道碎石结合前列腺电切术是治疗前列腺增生膀胱结石的有效方法.

Objectives To study the clinical effect of different surgical methods in the treatment of benign prostatic hyperplasia with bladder stone.Methods A retrospective analysis was done in 129 patients with benign prostatic hyperplasia with bladder stone in our hospital.The patients were divided into observation group and control group according to different operation methods.The observation group was treated with pneumatic lithotripsy combined with prostate resection,and the control group was treated with regular operation method.Therapeutic effect,the VAS score of pain and complications were compared between two groups.Results Taking stone time and operation time in the observation group was longer than the control group （P 〈 0.05）.Taking stone time,operation time,blood loss,bladder washing time,catheter time and hospitalization time,the average score of postoperative pain VAS score in the two groups were statistically significant（P 〈 0.05）.The incidence of postoperative complications in the two groups,including the distribution of infection,hematuria and hemorrhoids was statistically significant（P 〈 0.05）.The incidence of complications in the observation group was less than that in the control group.Conclusions Pneumatic lithotripsy combined with transurethral resection of prostate is an effective therapy for benign prostatic hyperplasia with bladder stone.
