活血解毒通脉汤结合尿激酶治疗下肢深静脉血栓的临床研究

Clinical Study of Huoxue Jiedu Tongmai Decoction Combined With Urokinase in the Treatment of Deep Venous Thrombosis of Lower Extremity

目的观察活血解毒通脉汤结合尿激酶治疗下肢深静脉血栓形成的临床效果。方法收集从2002年2月~2013年2月收治85例LEDVT住院患者,分为两组,对照组42例,通过患肢足背静脉途径给药给予尿激酶溶栓、祛纤治疗,并配合医用弹力袜等治疗;治疗组43例,采用活血解毒通脉汤结合西医治疗。观察患肢好转、显效、治愈情况,观察血管再通率、6月再闭塞、显效中位时间、出血并发症。结果治疗组、对照组血管再通率分别为93.48%和76.09%（P〈0.05）,6月再闭塞率分别为0.00%和13.04%（P〈0.05）,显效中位时间分别为9.62 d、14.02 d（P〈0.001）,出血发生率分别为2.17%和10.87%（P〈0.01和P〈0.001）。治疗组患者在血管再通、6月再闭塞率、和出血并发症等方面显示出了优势。结论经过活血解毒通脉汤配合尿激酶中西医结合治疗,可增加血管再通率,减少6月再闭塞、显效中位时间、出血并发症。中西药有机结合运用,辨病辨证相结合,充分发挥西药疗效快、中药审证求因、辨证施治及毒副作用少的优势,在提高此病疗效及治疗安全性方面,展现出广阔的前景。

Objective To observe the clinical effect of huoxue jiedu tongmai decoction combined with urokinase in the treatment of deep venous thrombosis of lower extremity. Methods From February 2002 to February 2013,85 cases of LEDVT patients,divided into two groups,42 cases in the control group,the limb dorsalis pedis vein administration given urokinase,removing fiber treatment,and cooperate with the medical stretch hose treatment,the treatment group of 43 cases,with huoxue jiedu tongmai decoction combined with western medicine treatment,observation of limb improved,markedly effective,cure,observe the recanalization rate,effective 6 months,the median time to occlusion,bleeding complications. Results In the treatment group and the control group,the recanalization rate were 93.48% and 76.09%（P 〈 0.05）,recanalization rate after six months were 0% and 13.04%（P 〈 0.05）,effective in time was 9.62 days,14.02 days（P 〈 0.001）,bleeding rates were 2.17% and 10.87%（P〈 0.01 and P 〈 0.001）. The treatment group in patients with recanalization,recanalization rate after six months,and bleeding complications showed obvious advantages. Conclusion After jiedu huoxue tongmai decoction combined with western medicine treatment of urokinase,can increase the vascular recanalization rate,reduce the june re occlusion,effective in time,bleeding complications. traditional chinese medicine and western medicine in organic combination of disease and syndrome differentiation,combined,give full play to efficacy of western medicine,chinese medicine soon permit requirements for the trial,and the side effects of treatment based on syndrome differentiation less advantage,to improve the therapeutic effect and safety,showing broad prospects.
