Analysis of 1076 Irrational Prescriptions in the Hospital′s PIVAS

目的了解攀枝花市中心医院静脉用静调配中心不合理医嘱的一般情况、规律,为减少临床给药错误,安全、合理用药提供参考。方法采用回顾性分析方法,对该院静脉用药调配中心2015年收集的1 076条静脉用药中不合理医嘱进行统计、分析。结果不合理医嘱主要为药物溶剂选择不当、浓度不当、药物选择不当、配伍禁忌、超说明书用药等。结论通过药师审核医嘱,提高了临床用药的合理性和安全性。

Objective To understand the general situation and rules of the irrational prescriptions in Pharmacy Intravenous Admixture Services of Panzhihua Central Hospital to provide a reference for reducing the errors in clinical administration, safety and rational use of drugs. M ethods Retrospective classification was used to analyze the statistics and analysis of 1 076 cases of unreasonable medical orders in intravenous medication in the hospital in 2015. Results Irrational prescriptions were mainly the drug solvent selection, improper concentration, the frequency of drug delivery being unreasonable, ultra manual medication, incompatibility and so on. Conclusion Through examining the prescriptions by pharmacist, the rationality and safety of clinical medicine usage is improved.
