不同剂量质子泵抑制剂治疗消化性溃疡临床对比研究

Clinical Comparison of Different Doses of Proton Pump Inhibitors in Treating Peptic Ulcer

目的探讨消化性溃疡患者采用不同剂量质子泵抑制剂治疗的临床疗效。方法选取2012年8月至2014年8月收治的消化性溃疡患者102例,随机分为A,B,C组,各34例,均采用质子泵抑制剂奥美拉唑静脉滴注治疗,临床单次应用剂量分别为40 mg,80 mg与120 mg,持续治疗4 d后观察3组患者的临床疗效与不良反应发生情况,并行统计学比较分析。结果 B组患者的临床症状疗效评估中总有效率为91.18%,胃镜检查疗效评估中总有效率为88.23%;C组患者的临床症状疗效评估中总有效率为94.12%,胃镜检查疗效评估中总有效率为91.18%,两组间比较无统计学差异(P>0.05),但均显著高于A组患者,差异有统计学意义(P<0.05)。同时,3组患者的不良反应发生率比较未见显著差异(P>0.05)。结论质子泵抑制剂奥美拉唑是临床治疗消化性溃疡的有效药物,高剂量用药时可发挥最佳药用效果,单次静脉滴注用药剂量80 mg时为最佳。

Objective To investigate the clinical efficacy of different doses of proton pump inhibitors in the treatment of peptic ulcer.M ethods 102 cases of peptic ulcer patients in the hospital from August 2012 to August 2014 were selected and randomly divided into group A, B and C, 34 cases in each group. All groups were given proton pump inhibitor omeprazole infusion therapy. After the clinical application of single doses of 40, 80, 120 mg, for 4 d of continuous treatment, the clinical efficacy and adverse reactions of these groups were observed and statistically compared and analyzed. Results The total effective rate in the clinical symptoms efficacy evaluation in group B was 91. 18%, the total effective rate in gastroscopy efficacy evaluation was 88. 23%; group Cwere 94. 12% and 91. 18%, with no statistical difference between groups（ P〉 0. 05）, but both groups were significantly higher than those of group A（ P〈 0. 05）. And there were no significant differences in the adverse reactions incidences among the 3 groups（ P〉 0. 05）. Conclusio n Proton pump inhibitor omeprazole is an effective drug in the treatment of peptic ulcer, its high dose can play the best effect, and 80 mg of single intravenous dose is the best choice.
