清肾排毒汤结肠透析治疗慢性肾衰临床观察

Clinical observation on the treatment of chronic renal failure by colon dialysis with clear-kidney detoxification soup

目的临床观察中药结肠透析治疗慢性肾衰竭的疗效。方法选取2013年1月至2015年3月大庆市中医医院肾病风湿科的慢性肾功能衰竭住院患者60例。治疗组和对照组各30例。治疗组应用清肾排毒汤结肠透析;对照组以常规结肠透析方法。观察治疗前后患者症状改善,肾功能变化情况及2种方法的疗效比较。采用SPSS13.0统计学软件进行统计分析,计量资料用均数±标准差(χ±s)表示,采用t检验,计数资料用χ2检验,以P<0.05为差异有统计学意义。结果 (1)治疗组尿素氮、血肌酐及血尿酸分别为(19±8)mmol/L、(412±26)μmol/L、(474±42)μmol/L;对照组分别为(14±6)mmol/L、(325±31)μmol/L、(384±57)μmol/L,与对照组比较显著降低,差异有统计学意义(t=4.26、2.85、5.21,均P<0.05)。表明清肾排毒汤结肠透析法可显著改善慢性肾衰竭患者肾功能。(2)治疗后两组中医症状积分均下降,治疗组症状积分为(9.4±1.9)分,较对照组(12.2±2.5)分降低,两组间比较差异有统计学意义(t=1.88,P<0.05)。(3)临床疗效比较,对照组显效2例,有效10例,稳定12例,无效6例,总有效率80%(24/30);治疗组显效4例,有效14例,稳定9例,无效3例,总有效率90%(27/30)。治疗组疗效优于对照组,两组比较差异有统计学意义(χ2=2.54,P<0.05)。结论单纯清肾排毒汤中药结肠透析对慢性肾衰在改善肾功能,中医症状及临床疗效方面均优于单纯的结肠透析法,可延缓疾病进展,值得临床推广。

Objective To observe effectiveness of colon dialysis with traditional Chinese medicine（TCM） for treatment in patients with chronic renal failure. Methods Sixty patients with chronic renal failure, in the department of rheumatism, Daqing city, from January 2013 to March 2015, were included in this study. Patients were divided into two groups： treatment group and control group, 30 cases in each group. The treatment group was treated with the renal detoxification decoction and the control group was treated with routine colon dialysis method. The improvement of symptoms and the changes of renal function were observed before and after treatment. The SPSS 13.0 statistical software was used for statistical analysis, and measurement data were expressed as mean and standard deviation（ χ ±s）. The count data were analysed by chi square test. P-values less than or equal to 0.05 were considered statistically significance. Results After treatment, levels of urea nitrogen, serum creatinine and serum uric acid were significantly improved[（19±8） mmol/L,（412±26） μmol/L and（474±42） μmol/L ]in treatment group compared with those of control group [（14±6） mmol/L,（325±31） μmol/L and（384±57） μmol/L], there was a significant difference between two groups（t=4.26, 2.85 and 5.21,P〈0.05）. After treatment, the symptom scores were significantly decreased in two groups. The symptom scores were significantly lower in treatment group（9.4±1.9） than those in control group（12.2±2.5, t=1.88, P〈0.05）. In control group, 2 cases were markedly effective, 10 cases were effective, 12 cases were stable, and 6 cases were ineffective. The total effective rate was 80%（24/30）. In treatment group 4 cases were markedly effective, 14 cases were effective, 9 cases were stable and 3 cases were invalid, the total effective rate was 90%（27/30）. The curative effect was significantly better in treatment group than that of control group（χ^2=2.54, P〈0.05）. Conclusion Traditional Chinese medicine colon dialysis of kidney detoxification soup has obvious curative effect on chronic renal failure at early and mid-term, which can delay the progress of disease and worthy of clinical promotion.
