摘要
目的:探讨64排螺旋CT血管成像对脑动脉瘤的诊断价值。方法:回顾性分析55例患者64排螺旋CT血管成像(CTA)和数字减影血管造影技术(DSA)的检查图像,以DSA检查结果为标准,对图像进行综合评价。计算两种检查方法的灵敏度、特异度、约登指数、似然比及Kappa值等指标,并进行准确率判断。结果:55例病例中,CTA发现47例48个动脉瘤,DSA发现49例50个动脉瘤;CTA诊断的脑动脉瘤的灵敏度、特异度和准确率分别为95.8%、100%和96.3%。结论:本研究揭示64排螺旋CT血管成像对脑动脉瘤的诊断灵敏度、特异度和准确率高,可用于筛查脑动脉瘤和指导治疗,但检查仍有一定的假阴性,不能完全取代DSA。
Objective:To explore the value of 64-layer spiral CT angiography(CTA) in the diagnosis of cerebral aneurysms.Methods: 64-layer spiral CT angiography and digital substraction angiography(DSA) imaging findings of fifty-five cases were retrospectively analyzed, using DSA interpretation as the reference standard, and the imaging were comprehensively evaluated. The sensitivity, specificity, Youden index, likelihood ratio and Kappa value were calculated and the accuracy was estimated.Results: Forty-eight cerebral aneurysms were found in 47 cases CTA, and fifty cerebral aneurysms were found in 49 cases. The specificity, sensitivity and accuracy of CTA were 100%, 95.8% and 96.3%.Conclusion: The 64-layer spiral CT angiography has characteristics of high specificity, sensitivity and accuracy for cerebral aneurysm. It is suitable for screening purposes and directing the treatment of cerebral aneurysm.
