自制透明牙托压低上后牙腭尖的临床研究

The effects of an artificial appliance on palatal tip intrusion

目的评价使用自制透明牙托压低伸长上后牙腭尖的临床效果,为正畸治疗伸长腭尖提供临床参考和依据。方法选择由种植科转诊的11例患者作为研究对象,因患者无法使用种植体支抗而采用自制透明牙托对伸长的腭尖进行压低。矫治前、后拍摄CBCT,利用软件进行三维重建、三维重叠及三维定点测量,并记录正畸压低前及压低后近、远中腭尖压入量,同时观察所压低牙齿及其他支抗牙齿的牙周和根尖状况。结果 11例患者伸长的上后牙腭尖均被成功压低,平均疗程为5个月,近中腭尖最大压低量为4.13 mm,远中腭尖最大为4.04 mm;被压低牙及支抗牙没有发现明显的牙根吸收和牙周变化。结论自制透明牙托有其独特的优势,是一种简单、有效压低伸长上后牙腭尖的临床方法。

Objective To evaluate the clinical effect of an artificial appliance on palatal tip intrusion,so as to provide clinical reference and basis for treating palatal tip intrusion. Methods 11 patients who were not suitable for the micro-implant were selected. The clinical effect of intrusion was evaluated by CBCT taken before and after treatment. The palatal tip distances were measured before and after treatment. Periodontal and apical conditions were observed. Results The average treatment time of all patients was 5 months.The comparison between pre-and post-treatment showed that the maximum intrusion was 4. 13 mm at mesial palatal tips and 4. 04 mm at distal palatal tips. No obvious root absorption or periodontal changes were observed in palatal tip intrusion with the artificial appliance.Conclusion This artificial appliance is effective in intruding palatal tip.
