比较影像学在颈动脉斑块评估中的研究进展 被引量:17

摘要 颈动脉粥样硬化斑块的破裂、栓塞及动脉管腔狭窄是引起缺血性脑卒中和短暂性脑缺血发作的重要危险因素。对颈动脉斑块易损性及颈动脉狭窄程度的早期评估、早期干预可以明显降低心脑血管急症的病死率和致残率。血管超声作为一种无创、无辐射、简便易行的检查手段,可作为颈动脉斑块检查的初筛工具,各种超声新技术的发展也在颈动脉斑块易损性的评价方面起了很大作用。CT血管造影在钙化斑块的检查优于其他影像检查方法,成像速度快,可作为其他检查方法的补充。磁共振成像软组织分辨率高,可清晰显示颈动脉管壁及管腔内情况,可对颈动脉斑块成分进行定性测量。目前,诊断颈动脉粥样硬化引起的管腔狭窄程度的金标准仍然是数字减影血管造影。综合分析各种影像检查手段的优缺点,选择患者最易接受,对临床治疗指导价值最高的检查方式仍然是比较影像学的重中之重。
