希氏束和希氏束旁起搏的研究进展 被引量:7

摘要 右心室心尖部起搏具有可操作性强、电极稳定不易脱落等优点,目前临床应用广泛。但许多研究表明右心室心尖部起搏并非理想的起搏位点,因为心尖部起搏时心室激动的顺序与生理性起搏相反,可致潜在的心功能恶化、瓣膜功能不良、心律失常,在特定人群中有较高的病死率。近年来开展较多的右心室流出道起搏,其获益仍有争议。运用减少心室起搏的算法能降低心室起搏比例,但对房室阻滞的患者,起搏不可避免。希氏束起搏理论上更接近生理性起搏,直接激动希氏束,使左右心室电和机械活动同步。近10余年来许多人探索了希氏束和希氏束旁起搏较其他起搏点在心功能、心脏再同步治疗(CRT)、心电图等方面的比较,本文就希氏束和希氏束旁起搏展开论述。
