重庆某三级甲等医院实施临床路径管理的效果评价

The Effect Evaluation on the Application of Clinical Pathway Management in a Class A and Tertiary Hospital of Chongqing

目的探讨临床路径的实施对促进医院运行效率提升的效果。方法采用SPSS22.0进行分析,选取输尿管结石、老年性白内障、腰椎间盘突出症和先天性室间隔缺损4个病种。将采用临床路径的337份病例作为实验组,传统方法诊疗的580份病例为对照组。结果将4个病种的患者的性别、年龄、治疗效果进行χ^2和t检验,P〉0.05,差异无统计学意义;实验组的住院天数比对照组的住院天数短,经独立样本t检验,P〈0.05。除了先天性室间隔缺损病种,实验组的平均住院费用为28678.50元,对照组为31653.79元,t=1.294,P〉0.05;其它3个病种总费用均显著下降。结论临床路径的实施不仅可以减少平均住院日、增加床位周转次数,还可以有效减轻患者的直接和间接经济负担,提高患者满意度。

Objective To investigate the application effect of clinical pathway to the improvement of running efficiency of the hospital. Methods SPSS22. 0 was adopted to make analysis, four kinds of diseases included ureteral calculi, senile cataract, lumbar disc prolapse, congenital ventricular septal defect were selected in this study. 337 cases of medical records who applied clinical pathways were taken as the experimental group, and 580 cases of medical records who received traditional method were taken as the control group. Results Chi-square and t test were performed on the gender, age, therapeutic effect of the patients with these four kinds of diseases, P〉0. 05, the difference had no significance. Hospitalization days of the experimental group were shorter than that in the control group, with P〈0. 05 by the independent sample t test. In addition to congenital ventricular septal defect disease, the experimental group the average hospitalization cost was 28678.50 yuan, the control group was 31653.79 yuan, t = 1. 294, P〉0. 05;The total cost of the other three diseases were significantly decreased. Conclusions The application of clinical pathway not only could reduce the average hospitalization days, increase bed turnover times, but also could effectively reduce the patient＇s economic burden directly and indirectly, and improve the satisfaction rate of the patients.
