射波刀治疗脑膜瘤疗效分析 被引量:3

摘要 总结射波刀治疗脑膜瘤的疗效。回顾性分析42例术后残留、复发或不适合手术的脑膜瘤的临床资料。首次应用射波刀治疗25例,术后残留和/或复发17例。均应用射波刀分次治疗,肿瘤的处方剂量16Gy^35Gy,中位值24Gy;等剂量曲线60%~87%,中位值80%。随访12个月~30个月,中位值20个月,术后患者症状好转32例,稳定6例,加重4例;复查MRI显示:2例肿瘤完全缓解,16例部分缓解,22例稳定,2例进展,肿瘤控制率95.24%。射波刀治疗脑膜瘤初步疗效满意,可提高脑神经对射线的耐受剂量,长期疗效有待进一步随访。 To summarize the therapeutic efficacy of radiotherapy with cyberknife for meningiomas. Clinical data of 42 patients with residual, recurrent or inoperable meningiomas were analyzed retrospectively. For the first time, 25 cases were treated with cyberknife, 17 cases were residual and / or recurrence after operation. Fractionated radiation by cyberknife was performed in all the patients. The median of irradiation dose was 24Gy, ranged from 16Gy to 35Gy in the surrounding tumors. The median percent of isodose curves was 80%, ranged from 60% to 87%. All the patients were followed up for median of 20 months, ranged from 12 to 30 months. The symptoms of the original were improved in 30 cases and unchanged in 8 and worsened in 4. 2 patients achieved complete response, 16 patients achieved partial response, 22 patients achieved stable disease and 2 patients achieved progressive disease according to MRI examination. The tumor control rate was 95.24%. Radiation by cyberknife can increase the dose the cranial nerve can tolerate. The preliminary results demonstrate that tumors could he controlled well, but long-term follow-up studies are needed.
