特殊部位硬膜下血肿的CT诊断

CT Diagnosis of Subdural Hematoma at Special Locations

目的 探讨大脑镰、小脑幕硬膜下血肿的CT特点。资料与方法 回顾性分析 32例大脑镰、小脑幕硬膜下血肿的CT和临床资料。结果 32例中大脑镰硬膜下血肿 2 0例 ,CT表现为内缘平直、外缘弧形或波浪形的带状高密度影 ;小脑幕硬膜下血肿 12例 ,CT表现为高密度片状影 3例、新月状影 3例、U形影 2例 ,累及大脑镰后部 4例 ,呈Y形或镰刀状。结论 大脑镰、小脑幕硬膜下血肿的CT表现具有特征性 ,CT复查对其诊断和鉴别诊断很有意义。

Objective To evaluate CT features of subdural hematomas at cerebral falx and tentorium of cerebellum.Materials and Methods CT findings and clinical data of 32 cases with subdural hematomas at cerebral falx or tentorium of cerebellum were retrospectively analyzed.Results Twenty cases had subdural hematoma at cerebral falx. On CT scan, the lesions presented as band shaped high density shadow with straight internal margin and waved external margin. The other 12 cases had subdural hematoma at tentorium of cerebellum. On CT scan, the high density lesions displayed patchy (n=3), crescent (n=3), 'U' shaped (n=2), or 'Y' shaped (n=4) in figuration. The formation of 'Y' shaped sign was due to the invasion of the hematoma into the cerebral flax.Conclusion Subdural hematomas at cerebral falx or tentorium of cerebellum carry certain characteristic CT features. CT re examination is of great value for the diagnosis and differentiation.
